The Research For The Reform Of The Urban Rural Basic Medical Insurance System In Chongqing Under The View Of Overall Urban—rural Development

The maintenance of social justice is a core value goal to build a coordinating urban and rural social security system. The whole urban and rural basic medical insurance system reform is in order to promote the urban and rural social members’ access to basic health services under the economic justice. The important social policy satisfying the needs of the social members’ basic medical treatment is a new social policy adjustment combining with the new changes of economic and social development. For a further and steady development of the Chinese cities and countrysides, the third plenary session of the 18 th emphasizes the mechanism of the integration of urban and rural development to accelerate the establishment of a fair and sustainable social security system in several major issue decisions about the comprehensively deepened reform. The basic medical insurance system, as one of the important ways to safeguard the people’s livelihood, is the direct embodiment of the social fairness, justice and sharing. It can break the stiff dual structure between urban and rural areas to promote the construction of the Chinese modernization smoothly. In China, for a long time, there are still some problems existing in the basic medical insurance system, such as: the basic medical insurance is separated into several parts according to the different areas and institutions. Even this system is showed with a fragmentary state resulting in the residents in the less developed and rural areas lose the basic right to equal access to basic health insurance services. Because of the poverty, the problems of difficulty and high cost can not be fundamentally solved.Besides, the expenditure of the insurance in primary medical treatment becomes bigger and bigger but the sustainability becomes more and more weakened, even the ability of the basic medical insurance management is not strong. All of these have always been the theoretical and practical problems for the government and academia.Chongqing is the only municipality directly under the central government in the west and the city of the comprehensive pilot reform between the urban and rural areas.The main goals are to break the urban and rural segmentation and narrow the gap to promote the integration of the urban and rural areas. And Chongqing has made a positive exploration and accumulated experience in the urban and rural basic medical insurance. For example: It first combines the basic medical insurance for urban residents and the new rural cooperative medical insurance system, which conforms the new basic condition for the urban-rural development in a new period. The integration of urban and rural basic medical insurance and the universal health insurance is the inevitable choice to break the fragmentary state of the system design.Therefore, the exploration of this kind of system integration provides a useful reference for the policy integration of urban and rural residents’ medical insurance system which was first put forward in 2016, also it’s the driving force of the writer to launch the research.The basic medical insurance system is the core content of social security as well as a irreversible step in balancing urban-rural development which relates to the social justice and people’s livelihood. With the urgent and epochal development of social construction, the research perspective of sociology is the new way closed to the essence of reform and topic for discussion, and this is also the significance of this paper. At the same time, the writer wants to find the problems in the modern basic medical insurance system through this paper, besides the writer explores the methods with the oriented problems so that this paper can offer some useful advice and reference about social policy to the governmental decision-making departments, and that’s also the practical significance of this research.This study welfare basic principle as the foundation of theory, sociology, social security is always the focus as a whole under the background of urban and rural social members of the basic medical insurance demand, balance urban and rural basic medical insurance system reform in chongqing as the breakthrough point, in the "structure- process- result" three-dimensional theory under the framework of analysis, literature research, historical analysis, case interviews and secondary data using the research method of chongqing in the thorough analysis of basic medicalinsurance system vicissitude, effectiveness, and face difficulties, on the basis of the top design, combined with the national macro decision-making and for the future social development and social transformation in chongqing in the basic medical insurance system reform, make a targeted countermeasures and basic anticipation.This paper is divided into five parts: the first part chapter 1 is introduction, which is mainly expounded for the purpose of this research, background, significance, value,main contents, research methods and the innovation points. The second part chapter 2contains the basic concepts and literature review. The third part is chapter 3, which mainly states and sorts out the vicissitude and current situation of the basic medical insurance system in Chongqing. The fourth part chapter 4 which analyzes the effect of the system and the existing problems.About the urban and rural basic medical insurance system reform, Chongqing starts early from three aspects of reforming the basic medical insurance of urban and rural areas. The system design and policy are integrated perfectly, the methods of the governance and management get normalized and strengthened to make some phased achievements. From the implementation of basic medical insurance system in urban and rural areas, the effect of the system goes well but it also exposes some problems: The first is the multilevel system is missing.The second is there exists a mismatch between the system supply and system demand.The third is the insufficient technical barriers lead to an unequal social services.The fourth is the lack of binding system results in a vacancy of the responsibility. The fifth part, chapter 5 puts forward innovation paradigm of the system through the author’s literature searching and interviews, as well as the shortcomings that exist in the current system. Even the writer puts forward the basic methods and specific policy recommendations to solve the problems, according to the basic concept and the key in the system design that we should stick to and grasp.
