In January, 2016, The State Council issued “a guideline on the integration of basic Medical Insurance for urban and rural residents”, integrated the new rural cooperative medical system for rural residents with the basic medical insurance subsidies for urban residents, which indicated that China is striding forward universal coverage progressively. In recent years, China’s medical insurance policy is reaching covering whole people, presenting strict requirement to basic level medical institutions. Governments at all levels are attempting to explore new modes of health authority fitting to the development of social economy and people’s living standards. Under this background, it is obviously that discussing about the impact—positive or negative—acted on management of basic level medical service by medical insurance for urban and rural residents means important theoretical values and practical significance.Based on the stakeholder theory, the article analyses the impact of medical insurance for urban and rural resident on management of basic level medical service in five sections. Introduction, the first section, put forward the theme and its value of research, defines several terms related to the article, sorts out relevant literature published in both domestic and foreign, and introduces the research methods, structure, invention and shortcoming.The article makes some introduction in section two, including the characteristics of the modes of universal coverage of health care insurance in developed countries, different kinds of medical service, three medical insurance for urban and rural resident systems and the development of basic level medical in China. After briefly analyzing the relationship between medical insurance for urban and rural resident and management of service, the article illustrates local medical insurance for urban and rural resident and its current situation of the management of basic level medical institutions.By means of describing medical insurance system’s supervision and management on basic level medical institutions, and the impacts of medical insurance for urban and rural resident on the medical and health system put into practice, the third section analyzed how medical insurance for urban and rural resident works on basic level medical institutions as a extrinsic pressure.Now for the influence acted on basic level medical institutions itself, there are 2 aspects to illustrate in section 4: one is the ways how revenue of basic level medical institutions were influenced by medical insurance for urban and rural resident; the other one is the ways how basic level medical institutions’ resource utilization——including infrastructure construction,was medical resource utilization and human resource system ——influenced by medical insurance for urban and rural resident.The fifth gives proposals about legislation, reform of health care system, methods of payment, supervision, talent management and involvement of business mode, etc.The last section summarizes the impact—positive or negative—acted on management of basic level medical service by medical insurance for urban and rural residents.The innovation of this paper lies in the following aspects: the article discusses the function that medical security system works on the development of basic level medical institutions, and its problems. The main point is the manage behavior of basic level medical institutions. It gives feasible proposals on the side of medical insurance to the development of basic level medical institutions. Elsewhere, it is the stakeholder theory that the article based on to analyze extrinsic and internal management of basic level medical institutions. Thus, the impact—direct or indirect, positive or negative—acted on management of basic level medical service by medical insurance for urban and rural residents is investigated respectively.
