Refining producing has much risk, such as high temperature, high pressure, inflammable, explosive and so on. It is easy to cause significant casualties and property losses. How to reduce the incidence of accidents, especially major accidents, is a long-term problems in refining and petrochemical industry. Therefore, studying on refining major accident warning, to enhance the level of safety management in refining and petrochemical industry, has important practical significance.Referencing Heinrich iceberg theory, every casualty accidents based on a large number of near-miss events, preventing major accidents must grab from near-miss events. Domestic near-miss events management system is not yet mature, it is difficult to collect the sample events. In this paper, through collecting refining major accident report, breaking major accidents into small segments of events using logic tree. These events are the near-miss events without other conditions. To solve the problem of near-miss events difficult to collect, through mapping major accident logical tree, putting forward 11 kinds of refining accident common forms, clearing its mechanism and key factors of all forms to prevent the accident. Studying the typical accidents cases to find out the key link in the process of the accident, creatively proposing refining sentinel events concept- refining sentinel events are the near-miss events that may cause great losses in the refining production. Establishing the sentinel event selection criteria, finding out sentinel events in a large number of accidents logical tree, then studying the relationship between sentinel events and major accidents on the logic tree. There also have some other important information in the logic tree. Combining the logic tree and the important information on the logic tree to form the sentinel event database. Integrating the enterprise's near-miss events reporting system, the sentinel events criteria, the sentinel events database, building out the refining major accident early warning system, helping enterprises to realize sentinel events among the huge number of near-miss events, warning the possibility of major accident forms, putting out targeting prevention countermeasures for the enterprise, preventing the occurrence of major accidents.
