Research background:Diabetic foot is a global health problem,2013 statistics show that the global diabetes patients about 382 million,while China ranks first in the world,the number has more than 100 million.Diabetic foot is the result of deterioration of diabetes,its manifestations of foot ulcers,and difficult to heal,resulting in infection,osteomyelitis,necrosis,and finally need to be amputation or cut toe.According to statistics from the United Kingdom and the United States,diabetes patients who require hospitalization for foot ulcers and foot infections account for about 20%of the total number of diabetic inpatients.The main cause of foot ulcers is peripheral neuropathy.Studies have found that abnormal increase in plantar pressure and abnormal distribution of plantar pressure is to predict the occurrence of diabetic foot ulcers independent risk factors,These indicators can also reflect the severity of foot peripheral neuropathy,increased aberrant foot pressure and abnormal distribution of plantar pressure can increase the specific parts of the foot ulcer risk,Therefore,reducing the abnormal increase in plantar pressure and improving the abnormal distribution of plantar pressure as a direction of effort.Specific diabetic foot foot is an adjunctive treatment tool,its main function is to reduce the abnormal increase in plantar pressure and improve the abnormal distribution of plantar pressure,the most widely used in foreign countries is the production of DARCO diabetes foot care shoes.Whether it is fully applicable to the needs of Chinese patients has no basis.Studies have shown that DARCO diabetic foot care shoes can relieve diabetic foot pressure in patients with plantar pressure,to correct the abnormal distribution of plantar pressure,thereby reducing the incidence of foot ulcers in diabetic patients.However,Darco shoes are designed for European and American patients.At present,there are many types of diabetic foot care shoes,manufacturers have claimed to have the function of foot care,but no corresponding experimental description,we can see from the perspective of biomechanical foot heel foot shoes for functional analysis,and Design and production in line with China's national conditions of diabetes foot footwear is a significant practical significance.Research propose and significance:To determine the abnormal increase of plantar pressure in diabetic patients and abnormal distribution of the region;On China's BMWALKER diabetic foot shoes has taken functional determination,and with the United States DARCO diabetic foot footwear contrast,while the two shoes subjective wear investigation and analysis.For the design and improvement of our population in accordance with the exercise prescription diabetic foot shoes to provide a basis for reference.Subjects and methods:10 patients with diabetes who had a potential risk of diabetes were confirmed by a specialist,And healthy young subjects 20 subjects,the subjects were tested with different shoes wearing foot pressure test and subjective feelings of evaluation,foot pressure test through the Novel Pedar-X plantar pressure distribution measurement system,the subjective comfort experience Through a questionnaire survey.Results:Diabetic patients wearing DARCO shoes and BMWALKER shoes can reduce the metatarsal at the pressure and impulse.Which DARCO shoes with better support at the arch,to share the forefoot and heel at the pressure;and BMWALKER shoes is by increasing the effective load area at the metatarsal,reducing its maximum pressure and impulse.Both shoes show different mechanisms to reduce the pressure on the forefoot.Healthy young people wearing DARCO and BMWALKER shoes can reduce the forefoot pressure and impulse and found the toe(T2-5)at the pressure reduction same as diabetics.Diabetes patients with maximum pressure,peak pressure,impulse and other indicators were significantly lower than healthy young people.The data show that this is due to the significant effect of the effective load area of the corresponding area of the plantar in diabetic patients greater than that of healthy young people.Wearing DARCO and BMWALKER foot footwear when diabetic patients and healthy young people's subjective comfort scores are generally higher than the benchmark shoes(old Beijing shoes).BMWALKER shoes score higher than DARCO shoes.Healthy young people feel more differences than indicators of diabetes.Conclusion:Two diabetic foot shoes have reduced the pressure at the forefoot and the impulse of the foot care function.Diabetic patients with the corresponding area of the effective area of the effective load area is greater than healthy young people,suggesting that this may be an indicator of diabetes diagnosis.Diabetes and healthy young people wearing DARCO and BMWALKER foot care subjective comfort score is generally higher than the benchmark shoes(old Beijing shoes).And BMWALKER shoes score higher than the DARCO shoes due to the high sensitivity of healthy young foot,the weight of the shoes,anti-skid and friction feel more refined than diabetic patients.
