Pelvic floor dysfunction is a common disease,seriously affecting middle and aged women's health and quality of life.In recent years,meshes are widely used in pelvic floor reconstruction surgery,which can act as a substitute for repairing weak or damaged fascia tissue,so as to treat pelvic floor dysfunction such as pelvic organ prolapse and stress urinary incontinence.Excellent pelvic floor repair meshes have good histocompatibility,allowing ingrowth of collagen fibers,soft and flexible,and can withstand a certain degree of tension or stress.Currently,study about pelvic floor repair meshes in abroad is comparatively mature,while relative product development in the domestic is lagging behind due to core technology deficiency.Therefore,self-developed meshes in domestic used in pelvic floor repair allow of no delay.In this regard,researches of material,structure design,preparation and performance of meshes used in pelvic repair were carried out.The main contents are as follows:Firstly,performance of the domestic polypropylene(PP)with two specifications and polylactic acid(PLA)were studied.Filament diameter,linear density,tensile properties,thermal properties and alkali resistance were tested for PP and PLA.Degradation performance in vitro of PLA monofilament was explored.In vitro cytotoxicity testing was done for PP monofilament.Secondly,knitting technology of pelvic floor repair mesh made of PP,pelvic floor repair mesh made of PLA and two kinds of knitting schemes A and B for sling used in stress urinary incontinence were figured out in the reference of the structures of the pelvic floor meshes and sling in the commercial market.Structure of composite mesh of PP/PLA was designed.Raschel warp knitting machine RSE4 was used to make abovementioned structures with 20E gauge.Thirdly,taking temperature and time as two variables,heat setting PP and PLA pelvic floor repair meshes respectively in the conditions of 9 experimental schemes.The influence of heat setting process on density,thickness,porosity,tensile breaking strength,bursting strength,bending stiffness and suture pull-off strength were discussed.Optimum heat setting process were determined for which PP pelvic floor repair mesh should be heated in the temperature of 130?with 15min and PLA pelvic floor repair mesh should be heated in the temperature of 130? with 10min.Fourthly,the heat-setting process of PP/PLA composite mesh was determined in the temperature of 130? with lOmin.Performance test of PP/PLA composite mesh was also carried out.Basic performance and mechanical properties of PP/PLA composite mesh,PP pelvic floor repair mesh and PLA pelvic floor repair mesh prepared by optimum heat-setting process were compared.Fifthly,sling was prepared in the way of A and B.Tensile strength and flexibility of sling were measured.Knitting scheme A was considered better for the preparation of sling.Through these studies,some rules and useful conclusions were obtained,which can provide guidance and reference for the manufacture and further study of meshed used in pelvic floor reconstruction.
