| Background:The preferred treatment of intertrochanteric fractures in aged patients(>80) is controversial. The purpose of the present study was to compare the outcomes of the Asian proximal femur intramedullary nail antirotation system(PFNA-Ⅱ) and artificial hip arthroplasty therapy among elderly intertrochanteric fractures.Methods:Collecting the clinical datas of 173 elderly patients with intertrochanteric fracture treated by PFNA-Ⅱ and artificial hip arthroplasty from January 2011 to January 2014 in our hospital. We statistically evaluated the operation time, blood loss, postoperative complications and outcomes according to Harris hip score.Results:Patients were followed up for a mean of 15.2 months (range, 7-48months).A total of 173 cases were selected, aged from 80 to 104 years. 59 cases in PFNA-Ⅱ group; 114 cases in arthroplasty group. There were no significant difference between two groups in general information and can be compared. There were significant differences between the two groups that PFNA-Ⅱ group had less operative time and amount of blood lost. There was no significant difference between two groups in the mean length of stay Harris hip score and postoperative complications.Conclusion:PFNA-II has the advantages of a simple operation, less operative time and less blood lost compared with arthroplasty for the treatment of intertrochanteric fractures. PFNA-II is an effective operation method even for the patients accompanied by severe osteoporosis. Systematical assessment and treatment for elderly trauma patients will ultimately lead to improvements in outcomes. |
