| Objective: Through dynamic contrast-enhanced MRI(dynamic contrast-enhanced magnetic resonance imaging,DCE-MRI)imaging parameters of the collection,comprising: a tumor morphology,edge cases,strengthen the manner and time-signal curve,Explore the traditional prognostic factors in breast cancer: the correlation between lymph node metastasis,tumor size,WHO(World Health Organization)histological grade and molecular prognostic indicators,Find the regular between breast cancer from the molecular level to the biological and pathological features and to the MRI imaging.We are Trying to assess the prognosis of breast cancer patients by radiographic parameters,and to some extent guide the choice of clinical treatment programs.Methods : Collected from January 2013 to December 2015 during a visit to Mianyang Central Hospital breast surgery in breast cancer patients,they were selected from 113 cases of patients meet the requirements of the patients,all of them were female,unilateral disease,a total of 113 lesions,aged 32 to 65 years,an average of 46.96 ± 7.801 years.113 cases of patients underwent preoperative breast MRI flat sweep + enhanced dynamic scanning,according to MRI breast Reporting and Data System(BI-RADS)describes morphological manifestations of breast cancer,such as: enhancement pattern(homogeneous enhancement,heterogeneous enhancement,ring enhancement),tumor morphology(leaf,no leaf),tumor edge(burr,smooth).Hemodynamic performance: time-signal curve(time-signal intensity curve,TIC)(according to Kuhl classification: Ⅰ,Ⅱ,Ⅲ type).Postoperative pathology test: tumor size,lymph node metastasis,WHO classification,and immunohistochemical detection of molecular biology factor,such as: estrogen receptor(estrogen receptor,ER),progesterone receptor(progesterone receptor,PR),proto-oncogene human epidermal growth factor receptor 2(human epidermal growth factor receptor 2,HER2),a marker of cell proliferation antigen(Ki-67).The test results by SPSS20.0 software for statistical analysis.Difference between groups were compared using the chi-square test;level data were compared between groups by rank sum test;Spearman rank correlation coefficient is used to describe the correlation between variables,r value determination positive and negative correlation;Logistic regression analysis was used to explore the relationship between radiographic parameters and prognostic indicators.Inspection level α=0.05,P <0.05 was been considered statistically significant.Results:(1)breast tumor morphology: There was a positive correlation between leaf situation and tumor size(r = 0.232 p = 0.014);Have no significant correlation with Lymph node metastasis(r = 0.136 p = 0.150);Have no significant correlation with WHO pathological grade(r = 0.178 p = 0.060);ER expression was not significantly correlated(r = 0.096 p = 0.312);Have no significant correlation with PR expression(r = 0.151 p = 0.111);Her-2 expression was not significantly correlated(r = 0.050 p = 0.598);Ki-67 expression was significantly correlated(r = 0.356 p <0.001).(2)the edge of the case: Have no significant correlation with Tumor size,lymph node metastasis,WHO grade;ER expression was significantly correlated(r = 0.305 p = 0.001);PR expression was positively correlated(r = 0.227 p = 0.015);Her-2 expression was not significantly correlated(r = 0.064 p = 0.502);Ki-67 expression was not significantly correlated(r = 0.032 p = 0.734).(3)enhancement pattern: Tumor size was no significant correlation(r = 0.035 p = 0.711);Lymph node metastasis was no significant correlation(r = 0.094 p = 0.322);With WHO pathological grade was a significant positive correlation(r = 0.307 p = 0.001);ER positive expression was significantly negatively correlated(r =-0.470 p <0.001);PR expression was significantly negatively correlated(r =-0.425 p <0.001);Have no significant correlation with the Her-2 expression(r =-0.050 p = 0.597);and Ki-67 no significant correlation(r =-0.012 p = 0.899).(4)TIC curve type: Have no significant correlation with all of the prognostic indicators.(5)tumor size: Have a significant negative correlation with ER expression(r =-0.278 p = 0.003);PR expression was negatively correlated(r=-0.241 p=0.010);Significantly positive correlation with Her2(r=0.388 p <0.001);A significant positive correlation with Ki-67(r = 0.553 p <0.001).(6)lymph node metastasis: Have no significant correlation with ER,PR,Her-2;But Ki-67 showed a significant positive correlation(r = 0.411 p <0.001).(7)WHO histological grade have no significant correlation with ER,PR;But Her2 showed a significant positive correlation(r=0.303 p=0.001),and a significant positive correlation with Ki-67(r=0.307 p=0.001).(8)Logistic regression multivariate analysis and chi-square test,rank sum test were all confirmed by a correlation between DCE-MRI parameters and breast cancer prognostic indicators,but there also has some difference between Imaging parameters and prognostic indicators.Conclusion: This study suggests a correlation between DCE-MRI parameters and breast cancer prognostic indicators,We can indirectly speculate prognostic indicators through the imaging features of Expression,and we also could conduct breast cancer prognosis and guide clinical treatment programs. |
