Background Diabetes is clinic syndrome caused by relative or absolute lack of insulin secretion and widely existed as a non-communicated chronic diseases.Due to the characteristic of illness complications,longterm course and higher disability rate,not only bring the patients in health threat,but it also causes an enormous burden on the economy. Globally,prevalence of diabetes carry heavy burden to the national health expenditure. As the aging of the population in China,the prevalence of diabetes is increasing year by year, which has widely become a threat to public health.Under the background that the current diabetes managing mode can’t fully adapt to the needs the elder patient with type 2 diabetes,we need to improve the current mode and for further to explore the new one.Based on the proved successful experience of diabetes managing mode research domestic and abroad and the disadvantages of classified managing mode and hospital-community managing mode,we try to develop a better mode to satisfy the needs of patient.Objective By comparing the results of two different mode,we analyze the effect, increased cost and feasibility of hospital-community-family three classed managing mode.Methods From September 2012 to March 2013,we take a random selection of 120 elder patients with type two diabetes,aging from 61 to 82.Collect the demographic data of patients(such as:sex,age,educational level,income,health insurance, marriage and living conditions),the diabetes knowledge level,the self-management level and the main indicator of blood index.We take 60 patients under the management of hospital-community-family managing mode and another 60 under the management of classified managing mode.After six months’ intervention,we analyze the difference of the diabetes knowledge level,the self-management level and the main indicator of blood index to compare the effect of the two managing mode.Results 1 Comparing the results of two managing mode after the intervention on the diabetes knowledge level:there is a rise on the diabetes knowledge level in the both of the group.Comparing the the results of two managing mode after the intervention on the self-management level:the six programs and the total score have been improved.The rational drug use, the self-text on blood index and the review frequency of the controlling group has been improved.However,the rest three programs has been declined slightly,but the difference is not significant,P>0.05.And the difference on total scores is not significant too.Comparing the the results of two managing mode after the intervention on the main indicator of blood index:both of the two group has declined and the difference is significant,P<0.05.Between them,the fall of the experimental group is higher than that of the controlling group.Conclusion The hospital-community-family three classed managing mode can improve the knowledge level of elderly diabetes efficiently and have a good improvement on the self-management behavior on patients.At the same time,it has a good function on reducing the numerical value of the main blood index.And its effect is better than that in the controlling group.During the time of the intervention,based on the classified managing mode,the extra rise on each patient is just 87.65 Chinese yuan.The cost-benefit ratio is of great value.
