| Objective:To explore the clinical significance on the detection of urinary podocyte marker protein podocalyxin(PCX)in children with hematuria or PNS.Methods:The research objects were obtained from 144 children with hematuria,50 children with PNS and 66 healthy children for control group.Hematuria group included 78 children with glomerular hematuria and 66 children with non-glomerular hematuria.The urinary PCX content of the hematuria and control group were quantified by turbidimetry.The urinary PCX content during active phase and release phase of 50 children with PNS were quantified by turbidimetry.The 24hUP and serum C3 content of the 50 children with PNS were quantified during active phase by turbidimetry.The serum content of ALB,BUN were quantified by colorimetric method and the serum content of Cr,UA,blood-fat of the 50 children with PNS during active phase were quantified by enzymic method.SPSS 16.0 software was used to analyze the data.Results:1.Results of hematuria group and control group1)The levels of PCX was 16.48± 18.95ng/ml in glomerular hematuria group,1.85±2.91 ng/ml in non-glomerular hematuria group,and 1.69±2.12 ng/ml in control group.The levels of PCX were significantly higher in the urine from glomerular hematuria group compared with those in non-glomerular hematuria group and the control group(P<0.01),but there was no obvious difference between non-glomerular hematuria and control group.2)The diagnostic method of urinary PCX content detection is highly concordant with that of urine flow cytometry(Kappa=0.647,P<0.01).3)The area under curve is 0.909,P<0.01,which suggest that detection of urinary PCX has highly accuracy on diagnosis and the cut of point for diagnosis is 5.73 ng/ml.The sensitivity is 0.808 and the specificity is 0.908.2.Results of PNS group1)The urinary PCX content during release phase(2.19±2.44 ng/ml)was obviously lower than during active phase(25.76± 17.95ng/ml)in children with PNS.2)There was positive correlation between the level of urinary PCX and 24hUP(r=0.768,P<0.01).3)There was negative correlation between the level of urinary PCX and plasma ALB(r=-0..755,P<0.01).4)There was positive correlation between the level of urinary PCX and BUN(r=0.607,P<0.01).There was no significant correlation between the level of urinary PCX and serum Cr.5)There was positive correlation between the level of urinary PCX and serum UA(r=0.633,P<0.01).6)There was positive correlation between the level of urinary PCX and serum CHO(r=0.579,P<0.01),positive correlation between the level of urinary PCX and serum TG(r=0.607,P<0.00)and positive correlation between the level of urinary PCX and serum LDLP(r=0.534,P<0.01).There was no significant correlation between the level of urinary PCX and serum HDLP.7)There was no significant correlation between the level of urinary PCX content and the level of serum C3.Conclusions:1.Quantification of urinary PCX is a useful marker to identify glomerular hematuria and non-glomerular hematuria.2.Quantification of urinary PCX can be used to distinguish active phase and release phase in PNS.3.Quantification of urinary PCX can be used to estimate the filtration function of glomerular and injury degree of renal tubule matrix,that’s to say,it can be used to judge the severity of PNS.4.Quantification of urinary PCX can be used to monitor hyper coagulabale state.5.The considerable value of urinary PCX is to forecast the prognostic of PNS. |
