Objective: To study the value of MRI diffusion-weighted imaging in the diagnosis of benign vertebral fractures and end-plate osteochondritis.Methods: To prospectively analyze conventional MRI images and DWI images which were from complete clinical data of 65 cases of benign vertebral fractures and end-plate osteochondritis patients.Thereinto,benign fracture group was divided into fresh fractures(group A1)17 cases and old fracture group(group A2)10 cases.end-plate osteochondritis group was divided into Modic type?group(group B1)11 cases,Modic type ? group(group B2)18 cases,Modic type ? group(group B3)9 cases.In the same time,the normal control group(group C)10 cases were set up.By measuring the b value which was respective 300s/mm2?500s/mm2 and 600s/mm2,got groups of ADC values,and the were pairwise compared.Result: Fresh fracture group(A1)showed long T1 and long T2 or equal T2 signal on conventional MRI images,high signal on STIR images and slightly high signal or high signal on DWI images.Old fracture group(A2)on conventional MRI images and DWI images showed signal in accordance with the normal vertebral body signals.End-plate osteochondritis Modic type?group(B1)showed long T1 and long T2 signal in the lesion area on conventional MRI images,high signal on STIR images and high or slightly high signal on DWI images.End-plate osteochondritis Modic type ?group(group B2)on conventional MRI images showed short T1 and long T2 signal in the lesion area,low signal on STIR and DWI images.End-plate osteochondritis Modic type ?group(B3)on conventional MRI images showed long T1 and short T2 signal in the lesion area,low signal on STIR and DWI images.When the b value was respective 300s/mm2?500s/mm2 and 600s/mm2,pairwise compared groups of ADC values which had been measured and discovered: when it is the same b value,group A1 and A2,B1 group,B2 group,B3 group,C group all have significantly statistical differences(p < 0.05);group A2 has no statistically different from C group(p> 0.05),but has significantly statistical difference from A1 group,B1 group,B2 group,B3 group,C group(p < 0.05);group B1 and A1,A2 group,B2 group,B3 group C group all have significantly statistical differences(p < 0.05);B2 group has no statistically different from B3 group(p> 0.05),but has significantly statistical difference from A1 group,A2 group,B1 group,C group(p < 0.05);B3 group has no statistically different from B2 group(p> 0.05),but has significantly statistical difference from A1 group,A2 group,B1 group,C group(p < 0.05).Conclusion: 1 ? In the study of benign vertebral fracture and end-plate osteochondritis,using this model when b value was 500s/mm2,not only can guarantee the quality of the DWI image,but also can reflect the real dispersion state of the lesions by measuring the ADC values.2? ADC value has an important value in differentiating benign fractures and end-plate osteochondritis,ADC values of fresh fracture vertebral body is higher than the ADC values of end-plate osteochondritis Modic type?.When the b value was 500s/mm2,the ADC value of fresh fracture vertebral body is(1.78±0.400 mm)×10-3mm2/s,but the ADC value of end-plate osteochondritis Modic type ? is(1.37 ± 0.162)×10-3mm2/s.Comparing the ADC values,there is significantly statistical differences between them.
