Objective: To investigate the correlation of CT features with the expression of MVD and CXCR4 in gastric cancer.Methods: 55 patients who were pathologically confirmed as gastric cancer underwent spiral CT scan and three phase enhanced scan a week before the surgery;tumor enhancement value and the stomach thickness were measured,tumor infiltration depth and metastasis of lymph node were described,and various signs were packet statistics;to detect the tumor microvessel density(MVD)and expression of chemotactic factor receptor(CXCR4)by immunohistochemical method after operation;the results were statistically analyzed finally.Results: 1.CT expression in gastric cancer:(1)the CT value of 55 cases of gastric tumors plain was 23.7~53.4HU,mean(36.93 ± 4.86)HU,the maximum enhancement value was 52.1 ~ 120.6HU,mean(86.79 ± 14.30)HU,enhanced amplitude was 18.90 ~ 84.03 HU,mean(49.86 ±14.28)HU;(2)in this study,the 55 cases gastric wall all have different degrees thickening,thickness was 5 ~ 33.7mm,mean(13.5± 5.3)mm;(3)there were 43 cases with tumor invasion to serosa,12 cases without invasion to serosa;(4)lymph node metastasis was found in 35 cases,no lymph node metastasis was found in 20 cases;(5)2 cases were found to with distant metastasis,53 cases without metastasis;(6)by the surgery and pathology,the accuracy of 55 cases of gastric cancer preoperative T,N and M staging with spiral CT was 78.18%,76.36%,66.67%,and postoperative pathological staging coincidence rate was 89.09%;the staging of spiral CT and postoperative pathological have good consistency(kappa = 0.823,P < 0.05).2.The count of tumor MVD was 12 ~ 69 / HP,mean(43.25 ± 17.63)/HP;there was a linear positive correlation between CT amplitude and MVD in gastric cancer(r=0.638,P<0.05),the regression equation was Y=3.979+0.807 X.The MVD with lymph node metastasis of were higher than those without lymph node metastasis(t=2.765,P < 0.05);the gastric wall thickness and tumorous gastric infiltration depth were not correlated with expression of MVD(P > 0.05).3.Expression of CXCR4 was positive in 24 cases,21 cases were negative in 55 cases of gastric cancer;the positive rate of expression of CXCR4 in lymph node metastasis group was higher than that of the group without lymph node metastasis(c2=13.48,P<0.05);the positive rate of expression of CXCR4 with serosal invasion was higher than that without serosal invasion(c2=5.337,P < 0.05).Conclusion: 1.The CT enhancement extent and lymph node metastasis carcinoma of castric cancer can reflect the expression of MVD can be used as an index to predict the expression of MVD,so as to evaluate tumor angiogenesis.However the depth of invasion of gastric cancer and gastric wall thickness can not reflect the expression of MVD in cancerous tissues.2.Lymph node metastasis and invasion of gastric cancer can reflect the expression of CXCR4 in cancer tissue,which can be used as an important index to judge the invasion,and metastasis of gastric carcinoma.
