Objective: To determine the value of spiral CT (SCT) in staging of bladder cancer. To evaluate the correlation of between three-phase enhanced SCT features, enhancement degree of the lesion and microvessel density (MVD), proliferating cell nuclear antigen (PCNA) in bladder cancer.Methods: Thirty-four patients with bladder cancer diagnosed by fibercystoscope or operation. All of them were examined by SCT, and the preoperative staging of T were used based on the finding of SCT. MVD and the expressions of PCNA were examined immunohistochemically using SP techniques and examined by DAB coloration. The results of three-phase enhanced SCT features were compared with those of surgical and pathological examination in all cases.Results: (1)30 cases of 34 patients were correctly staged with T, the accuracy was 85.3%. (2)The difference between average CT attenunation, MVD and PCNA in different histologic grade cancer lesion was statistically significant(P<0.05)。 A positive correlation was found between the CT enhancement and MVD(r=0.614,P<0.01),PCNA(r=0.632,P<0.01)。 (3)The positive expression of MVD and PCNA in the tumors that appear lobulate was higher than in margin regularity (P<0.01 )。 MVD was higher in groups of tumor whose diameter is larger than 2cm than in those of tumor whose diameter is less than 2cm(P<0.01); the expression of MVD and PCNA was higher in groups of tumor with central necrosis, with calcified wall, and with invasion than in those of tumor without central necrosis, without calcified wall, and without invasion (P<0.01)Conclusion: (1) Spiral CT is better method of depicting the bladder cancer, it allows for accurate depicting and staging of the bladder cancer , especially detecting the invasion of lateral wall of urinary bladder. However, the value of SCT for detecting mucous layer invasion and proper layer invasion is limited. (2) The expression of MVD and PCNA is a biological label of malignancy. The degree of enhancement correlated closely with the blood supply and MVD PCNA of the bladder cancer. Spiral dynamic CT scan can demonstrate the enhancement characteristics of the bladder cancer. (3)The segmental SCT features correlated with MVD and PCNA, that could be a useful prognostic predictor in bladder cancer invasion and aversion.
