Efficacy Analysis Of The Surgical Treatment Of The Syndesmosis Tibiofibularis Injury Of Ankle Fracture

Objective: To discuss the curative effect and relative merits about the treatment through syndesmosis tibiofibularis fixed screw and Endobutton system for ankle fracture.Method: During December 2013-November 2015,our hospital treated 38 cases of patients who have ankle joint fracture with distal tibiofibular syndesmosis injury.According to the method of surgery,the patients were divided into distal tibiofibular treatment group and Endobutton system treatment group.There are 24 males and 14 females.The age between 18~70years old,the average age is 38.55±12.32 years old.The fracture classified though Danis-Weber,there are 17 cases of SER type,6 cases of SA type,10 cases of PER type,5 cases of PA type.The Endobutton system treatment group has 13 cases and the distal tibiofibular fixed screw treatment group has 25 cases.We observed the operation time,intraoperatve blood soss,duration of hospital stays and the function of ankle joint after surgery.The patient did the clinical follow-up at the 1st,3rd,6th,9th,12 th after hospital discharge,had a X-ray on adem position of ankle joint.We made the assessment of curative effect according to the AOFAS ankle joint function scoring system when 12 months after the surgery.Result: The 38 cases of patients acquired the visit and assessment.The operation time of distal tibiofibular fixed screw treatment group is 35 ~ 82 min,the average time is45.36±11.22 min.The intraoperatve blood soss is 52 ~ 108 ml,the average value is60.27±13.02 ml.The duration of hospital stays is 7~10days,the average value is11.80±2.16days;Te operation time of Endobutton system treatment group is 63 ~118min,he average time is 83.15±12.32 min.The intraoperatve blood soss is 83 ~135ml,the average value is 86.40±14.16 ml.The duration of hospital stays is7~10days,the average value is 8.56±1.42days;Here are the assessments according to the AOFAS ankle joint function scoring system when 12 months after the surgery.the efficacy of 10 cases was excellent,8 cases was good,5 cases was moderate and 2 case was poor in the group using distal tibiofibular fixed screw,(The good rate is 72%);the efficacy of 7 cases was excellent,4 cases was good,1 case was moderate and 1 case was poor in the group using Endobutton system.(The good rate is 84.5%)Conclusion: Both of the two treatment groups obtained satisfactory therapeutic results.The amount of bleeding during the surgery of distal tibiofibular fixed screw treatment group is less than Endobutton system treatment group.The duration of hospital stays of Endobutton system treatment group fewer than distal tibiofibular fixed screw treatment group.There is a visible difference between the curative effect of two treatment groups after the surgery(P=0.03<0.05),they got the statistical significance.
