Objective:(1)To investigate the level of plasma Immunoglobulin E(IgE),Tryptase,Tumor Necrosis Factor-α(TNF-α)in Acute Coronary Syndrome(ACS)patients.(2)To investigate the relationship among plasma IgE,Tryptase,TNF-α and the degree of coronary artery’s stenosis.(3)To investigate the correlation among IgE,Tryptase and TNF-α mutually.Method: 70 patients diagnosed ACS were enrolled in this research who were admitted in Department of Cardiology of the second affiliated Hospital of Dalian Medical University from August 2015 to February 2016.According to the level of myocardial enzymes,70 patients were separated into AMI group(22 patients)and UA group(48 patients).57 outpatients diagnosed CHD by coronary angiography before were also enrolled in this research as stable angina pectoris group(SA group).44 patients were selected as the control group whose coronary angiography were normal.Meanwhile, according to the level of Gensini score,ACS group patients were divided into low score group(≤30,n=38)and high score group(>30,n=32).All patients were tested for blood and biochemical examination in the morning, the next day after admission.Tryptase and TNF-α were detected by ELISA kit in Shanghai corporation. All patients’ basic clinical date and outcome include Gensini score were amply recorded.SPSS 17 software were used for statistical analysis.Result: 1.The difference of age, smoking, diabetes, hypertension, fasting blood-glucose among the control group,SA group and ACS group were all statistically significant(P<0.05),while sex, triglyceride BMI,LDL,HDL,cholesterol among these groups were not statistically significant(P>0.05).2.Compare with SA group and control group,Plasma levels of IgE was significantly higher in ACS group(P<0.05),but there was no statistical difference between SA group and control group(P>0.05).Compare with SA group and control group,Plasma levels of Tryptase was significantly higher in ACS group(P<0.05),and there was statistical difference between SA group and control group(P<0.05).Compare with SA group and control group,Plasma levels of TNF-α was significantly higher in ACS group(P<0.05),and there was statistical difference between SA group and control group(P<0.05).3.Compare with SA group and control group,Plasma levels of IgE was significantly higher in AMI group and UA group(P<0.05),but there was no significant difference between AMI group and UA group(P>0.05).Compare with SA group and control group,Plasma levels of Tryptase was significantly higher in AMI group and UA group(P<0.05),but there was no significant difference between AMI group and UA group(P>0.05).Compare with SA group and control group,Plasma levels of TNF-α was significantly higher in AMI group and UA group(P<0.05),but there was no significant difference between AMI group and UA group(P>0.05).4.The difference of age,HDL,hypertension, triglyceride and IgE between low score group and high score group were all statistically significant(P<0.05),while BMI,LDL,sex, smoking, fasting blood-glucose,diabetes,cholesterol,Tryptase,TNF-α in these groups were not statistically significant(P>0.05).Through logistic regression analysis of risk of high gensini score,we found that in addition to age and HDL,IgE has obvious relationship with the risk of high gensini score(OR=1.009 95%CI:1.002-1.015),while there was no correlation among Tryptase,TNF-α with the severity of coronary artery lesions.5.Through logistic regression analysis of risk of ACS,we found that in addition to fasting blood-glucose,IgE,tryptase and TNF-α have obvious relationship with risk of ACS(OR=1.028 95%CI:1.015-1.041,OR=1.004 95%CI:1.002-1.007,OR=1.013 95%CI:1.007-1.018).6.There were positive correlation among IgE,tryptase and TNF-α mutually. Conclusion: 1.Plasma IgE,tryptase and TNF-α all increased in ACS patients.Testing IgE,tryptase and TNF-α in the sametime maybe useful for ACS diagnosis in the clinic.2.IgE maybe a new biomarker to predict the severity of coronary artery lesions, while there was no correlation among Tryptase,TNF-α with the severity of coronary artery lesions.3.There was positive correlation among IgE,tryptase and TNF-α mutually. It indicate that IgE may activate mast cell to promote the progression of atherosclerosis.
