Objective: This study selected different etiology and severity of chronic heart failure patients, though determinating plasma B-type natriuretic epeptide(BNP), to investigate the correlation between plasma BNP levels and the different etiological factor and degree of chronic heart failure, which can provide some help for the diagnosis and the degree of chronic heart failure(CHF).Methods : The study selected 749 cases of CHF from 2012 to 2015 in Cangzhou People's Hospital. 353 males and 396 females. Including criterion according to the causes of heart failure: 1)dilated cardiomyopathy( DCM);2)ischemic cardiomyopathy;3) hypertensive cardiopathy; 4)rheumatic valvular heart diseease; 5) cor pulmonale; 6) hypertrophic cardiomyopathy(HCM); 7) congential heart disease. In addition, 50 healthy adults were selected as the control group, including 26 males and 24 females. Detection index: After admission, all the selected patients were recorded in detail, including name, sex, age, family history and plasma BNP. With New York Heart Association(NYHA), the degree of heart failure was divided into I ~ IV. The blood of the elbow was extracted immediately and measured the concentration of plasma BNP by chemiluminescence method. At the same time, appling phillips IE33 S4 in patients with cardiac ultrasound probe to measure bout size and heart function of heart cavity. All patients determinated left ventricular ejection fraction(LVEF) by echocardiography and cardiac function examination, in order to explore the correlation between plasma BNP levels and left ventricular ejection fraction(LVEF), and to observe the correlation between LVEF and the severity of heart failureand, which can get the relationship between plasma BNP levels and different degrees of heart failure. In addition, the relationship between BNP and heart failure was observed by measuring the plasma BNP levels of different causes of heart failure.Results 1 The clinical data(age, gender, etc.) of the patients in each group were not statistically significant(P>0.05) between the experimental group and the control group. 2 Comparison of plasma BNP levels in patients with chronic heart failure of different etiology Mean 2.1 Dilated cardiomyopathy patients with plasma BNP levels higher than other 6 groups,with ischemic cardiomyopathy group following, significantly higher than the other five groups. The etiology of chronic heart failure patients with NYHA class increased and obvious difference(P < 0.05). 2.2 Pulmonary heart disease group in NYHA class I-II, plasma BNP mean and rheumatism valvular heart disease group, was significantly lower than the other five groups, the difference is statistically significant(P < 0.05), but in NYHA class IV was significantly increased, second only to the expansion cardiomyopathy group. 2.3 Plasma BNP levels were significantly lower in NYHA group than in other groups, the difference was statistically significant(P<0.05). 3.Correlation between plasma BNP levels and LVEDD, LVEF levels. 3.1 With the increase of the level of BNP increasing, the concentration of LVEDD gradually increased.(P<0. 001) 3.2 With the increase of LVEF level, the concentration of BNP gradually decreased.(P<0. 001). in cardiac function decompensated(NYHA class I), the plasma level of BNP elevated dilated cardiomyopathy and ischemic cardiomyopathy plasma BNP levels in patients with significantly higher than that of the control group(P < 0.01); pulmonary heart disease and rheumatic valvular heart disease BNP level and control group compared no statistical significance(P > 0.05).Conclusion:BNP level was significantly higher in patients with chronic heart failure, and patients with worsening heart function, plasma BNP levels gradually increased, suggesting that BNP diagnosis and severity of cardiac function in chronic heart failure have a certain objective evaluation of clinical value, especially for the early diagnosis of heart failure has clinical value. In this study we found that there are some differences between the degree of plasma BNP in chronic heart failure patients with different causes.
