Objective:To observe the efficacy of removal of sclerostin by peritoneal dialysis(PD) in end stage renal disease(ESRD) patients, compare the difference of the clearance of sclerostin by PD or hemodialysis(HD), thence to provide basis for choosing renal replacement treatment mode; To observe the relationship of the sclerostin in serum and PD effluent fluid with serum creatinine(Scr),calcium(Ca), phosphorous(P),papathyroid hormone(PTH), alkaline phospatase(ALP), and other relevant clinical factors in ESRD patients, so that to explore its role in the pathogenesis of CKD-MBD and provide basis for the diagnosis and treatment of CKD-MBD; To compare the differences among ELISA kits from different manufacturers for detecting serum and PD effluent fluid sclerostin, therefore to provide information for choosing sclerostin ELISA kits.Methods:Subjects were all from Jiangxi Provincial People's Hospital: 15 cases of HD,27 cases of PD(including serum and PD effluent fluid), and 15 healthy volunteers as control. Sclerostin was determined with 5 ELISA kits from different manufacturers(R&D,CUSABIO, Shanghai Hengyuan, Elabscience, Abcam); i PTH by radioimmunoassay; Ca, P, Scr, albumin by HITACHI automatic biochemical analyzer;hemoglobin(Hb) by blood analyzer. The relationship of serum and PD effluent fluid sclerotin and other results was analyzed. The differences of detecting sclerostin among ELISA kits from different manufacturers were compared.Results:(1)Compared with the control group: levels of serum sclerostin(determined with ELISA kits from R&D,CUSABIO,shanghai hengyuan),P,IPTH,Scr in HD groups were higher, while levels of Ca,HB,albumin(ALB) were lower(P<0.05); Levels of serum sclerostin(R&D,CUSABIO) P,IPTH,Scr in PD groups were higher, while levels of Ca?HB?ALB were lower(P<0.05).(2)Compared with HD group:levels of sclerostin(R&D,CUSABIO,ShanghaiHengyuan)in PD group were lower(P<0.05).(3) In PD group,statistically different levels of sclerostin between serum and PD effluent fluid was only detected with ELISA kit from R&D, with serum sclerostin higher than that in PD effluent fluid(P<0.05). Among these five ELISA kits from different manufacturers, the differences of levels of scleristin in different groups measured with ELISA kits from R&D ? CUSABIO and shanghai hengyuan had statistical significance, while no statistical significance in data obtained with Elabscience kit, Abcom kit did not detect any sclerostin in all groups.(4)Correlation analysis : serum sclerostin(R&D) in control group was positively correlated with age(r=0.733,p=0.0019)?P(r=0.5388,p=0.038)and i PTH(r=0.6674,p=0.0066); serum sclerostin(CUSABIO) in HD group was negatively correlated with Ca(r=-0.7684,p=0.0008); serum sclerostin(R&D) in PD group were positively correlated with Ca(r=0.507,p=0.0.007)and negatively correlated with i PTH( r=-0.498, p=0.008); PD effluent fluid sclerostin(R&D) was positively correlated with Ca(r=0.391,p=0.044)and that(CUSABIO) was negatively correlated with ALB(r=-0.383,p=0.049). For PD effluent fluid sclerostin determined with kits from R&D, CUSABIO, shanghai hengyuan, level of sclerostin by R&D kit was positively correlated with that by CUSABIO kit(r=0.535,p=0.004), the rest had no correlation.Conclusions:The levels of serum sclerostin in ESRD patients with PD and HD were higher than that in healthy people,; PD removed more serum sclerostin than HD, this could provided the basis for CKD patients to choose the dialysis mode. There were differences of detecting serum and PD effluent fluid sclerostin among ELISA kits from different manufacturers, this could be a reference for future researchers..
