| Objective By summarizing and analyzing the research results of video head impulse test of vestibular migraine patients,we want to dexplore the clinical value of diagnosis and treatment and in patients with vestibular migraine.Methods The control group includes 40 healthy people who come to our hospital for doing a routine checkup and no history of dizzines.Between September 2015 and February 2016 a total of 41 vestibular migrainen patients coming from Dalian Central Hospital hospital of Otolaryngology Head and Neck Surgery outpatient treatment center were enrolled in this study,collecting theirs detailed medical history,taking the test of pure tone audiometry,acoustic immittance measurement,otoacoustic emission,vHIT and other tests,vHIT gain value and saccadic eye movement results(positive or negative)as the detection index,compared to the differences between the two groups and within group vHIT results,and analysis vestibular migraine patients on both sides of three semicircular canal injury results characteristics.Results All of vHIT test results were in control group within the normal range,the average gain value of left and right of superior lateral and posterior semicircular canal on both sides respectively were 0.89±0.14 、 0.92±0.12;0.90±0.13 、 0.93±0.12;0.92±0.11、0.91±0.12,there is no statistically significant difference in the group(t1=-0.22,t2=-0.23,t3=0.08,P>0.05).The abnormal results of group of vestibular migraine have 14 cases(34.1%),Comparing semicircular canal’s function of vestibular migraine group and the control group,ang using the X~2 test,there is statistcally signficant difference(X~2=16.50,p<0.05),including 2 cases(14.3%)who only performed positive glance,4 cases(28.6%)who only have an abnormal saccadic gain and both abnormal saccadic gain value and positive glance had 8 cases(57.1%)in the vestibular migraine group.13 patients who were complaining most of sense of balanced instability coming to our hospital and taking Romberg test dump to a side after this test in all 41 patients,and the rest of the 28 patients made up smooth group,in whitch there were 9cases(69.2%)who had abnormal vHIT test result,Compared with the stable group of abnormal results,there was the significant difference between the two groups(X~2=10.42,P<0.05),moreover 8 cases(61.5%)had the unilateral abnormal results in the vHIT test results,1 patient(7.7%)had bilateral abnormal result,Compare the dumping and stable group of unilateral VHIT abnormal results,so dumping patients of vestibular migraine who sufferers vHIT unilateral abnormality rate was significantly higher than patients without dumping(X~2=11.68,P<0.05),However,dumping group bilateral VHIT abnormal results compared with the stable group,dumping and no dumping of vestibular migraine sufferering in bilateral vHIT had no obvious difference in the abnormal results(X~2=0.004,p>0.05).Conclusion 1.vHIT displays that part of vestibular migraine patients has an association;with vestibular dysfunction.2.vHIT may provide a complementary objectivity diagnostic method to diagnose patients with vestibular migraine.3.That vHIT is detected by vestibular migraine patients can offer possible targeting diagnosis for vestibular migraine patients,in order to provide directed recommendations for guidance for later stage ofvestibular rehabilitation vestibular rehabilitation.4.vHIT provides a theoretical basis to making the diagnosis and treatment process of vestibular migraine patients. |
