Objective To analyze vestibular function changes in patients with vestibular migraine(VM)through various laboratory methods to summarize the laboratory features of patients with vestibular migraine to facilitate the diagnosis of vestibular migraine and the differential diagnosis of other vestibular diseases.MethodsFrom January 2017 to July 2017,43 patients diagnosed as vestibular migraine and 30 healthy volunteers were collected in otorhinolaryngology head and neck surgery in Anhui Provincial Hospital.Pure tone audiometry,acoustic impedance,caloric test,ocular vestibular evoked myogenic potential(o VEMP),cervical vestibular evoked myogenic(c VEMP)and video head impulse test(v HIT)were used to analyze the laboratory results of the cases and control groups.Resluts In the case group,there were 5 males and 38 females,with an average age of 48.98 years.The onset age was mainly concentrated in 40-50 years old,and the duration of vertigo attack was mostly 39.5% in 1-24 hours.Of the 43 VM patients,10(23.3%)had pure tone audiometry abnormalities,mainly high-frequency hearing loss(20.9%),14 cases(32.6%)of hot and cold water test abnormalities,18 cases(41.9%)of abnormal o VEMP,Abnormal horizontal gain in 14 cases(32.6%)of v HIT,and compensatory sac results in 13 cases(30.2%)of v HIT were abnormal;in 30 cases of normal group,pure tone audiometry was all normal,3 cases(10.0%)of cold and hot water Test abnormalities,abnormal o VEMP in 5 cases(16.7%),abnormal horizontal gain in 1 case(3.3%)in v HIT,and compensatory sac results in v HIT were all normal,and the abnormal rate in VM group was significantly higher than that in normal group(P<0.05).Among them,the abnormal ratios of amplitude ratio(AR)and interaural asymmetric ratio(IAR)of o VEMP were highest,41.9% and 34.9%,respectively.Among 43 VM patients,the extraction rate of o VEMP was 83.7%.The incubation period of N10-P15 in the case group was longer than that of the control group.The difference was statistically significant,and the amplitude ratio and asymmetry between ears were significantly higher in the control group with significant differences.(P<0.05);The c VEMP extraction rate was 90.7%.The amplitude of the case group was significantly lower than that of the control group.The amplitude ratio and asymmetry ratio between ears were not significantly different from the control group.Conclusions Patients with vestibular migraine are more common in women and the accompanying vestibular symptoms last very long.Vestibular dysfunction may be common among vestibular migraine patients in any degree.The tests such as pure tone audiometry,caloric test,o VEMP and v HIT in the horizontal gain,compensatory saccades help to diagnose VM and distinguish other vestibular diseases.Compared with c VEMP,o VEMP is more helpful for the diagnosis of vestibular migraine,especially in o VEMP,the latency is prolonged,and the amplitude ratio and the asymmetry ratio between ears are increased.
