Objective With the development of society, there are more and more trauma patients.In order to be able to make a reasonable assessment of the injury and to help clinicians to develop areasonable treatment plan, trauma scoring tool such as Severity Score Injury is widely used in clinical practice. On the basis of ISS, revised injury severity score(RISS) was proposed by Zongkui Tan, which made a great contribution for the treatment of trauma patients,and it has been praised or cited by many scholars. But because of the need of manual operation, the work efficiency is solow that cannot be applied to alarge amount of patient’s processing, furthermore, it is affected by the subjective factor, which seriously affected its clinical use. In this study, the RISSs oftware and trauma database were compiled, and applied for treatment of fracture patients and analysis of the basic characteristics of patients with bone trauma in Wuhan General Hospital of Guangzhou Command to explore the feasibility of its application in the field of bone trauma, and to provide a theoretical basis for the treatment of trauma in the late.Method 1.Based on RISS calculation formula, the RISS software was prepared in Power Builder 9; 2. 312 cases of trauma patients in Wuhan General Hospital of Guangzhou Command from January 2014 to February 2014 were analyzed by RISS software and manual RISS retrospectively, and the evaluation results were compared with the treatment out come of each department, in order to verify the accuracy and the validity of the software,the scope of application and the ability to deal with the assessment of the volume of the wounded and the establishment of the evaluation database; 3.RISS software was used to evaluate the condition of bone trauma patients during the period of December 2015 to June 2014.And according to the different levels of injury to establish specific treatment options, guidance for treatment; Preliminary construction of bone trauma database. Cause injury, damage level, damage location for the conditions in the database query. RISS software was used to analyze the basic characteristics of patients with trauma in our hospital, and to explore the feasibility of this software in the treatment of patients with trauma and data analysis.Result 1. The RISS software was authored successfully. The software was intuitive and easy to operate, docking with HIS, PACS system program, and can directly check medical records system, relevant information of patients in inspection system. The software was divided into 3 interfaces : the login screen, the trauma score entry interface, the trauma score query interface. Patient basic information and injury assessment results were automatically saved into the scoring database. The basic information of patients can be input through the query interface. Condition setting and filtering query can be done according to user needs. The injury and assessment results were recorded from the database. 2.Through verification, we find that: in comparing the evaluation time, the average time for manual calculation of patients in each group was (53.75+4.85) minutes, the average time for the software calculation was(24.83 + 3.38) minutes, the difference of which was statistically significan t(t =1.69,P=0.000). In the assessment of the number of mistakes, the avera genumber of manual calculations for each group(2.25+1.55), the average number of software computing(0.67+0.99), the difference was statistically significant(t=3,P=0.007). All errors of software were human operator error, not calculation errors. All of the patients who used the software can be queried in the database by different index conditions, and the database was set up successfully. 3.RISS software had guided treatment of 1467 cases of bone trauma. By developed treatment programs and guided clinical treatment according to the different levels of injury,this group of patients were cured and discharged, with no patient died. Through the software retrieval and analysis, we found that patients with moderate injuries were the most, followed by serious injuries, more serious injuries and minor injuries were less, no dying patients; The fracture site consists of spinal fractures, pelvic fractures, upper limb bones(including upper limb fractures) and patients with lower extremity fractures; Causes of in jury include traffic accident injury, high falling injury, crush injury, fall injury and strike, among them, traffic accident injury and high falling in jury were the main components, which accounted for 50.37%,26.65%.Conclusion Preliminary application of the RISS software in patients with fracture,has also been tested confirm that the function of this-group-designed RISS software can completely replace the function of RISS,and it has the advantages of high efficiency and precision,less interference from the subjective factors. RISS software is applied to asses the damage of variou s parts of the body, especially in the treatment of large quantities of multiple injuries; At the same time, the establishment of the database is the basis for the analysis of injury epidemiology, which expand the RISS function, with strong clinical practicality.
