Objective:To explore a new microimplant anchorage traction aid pulling out mesioangular or horizontal impacted mandibular third molars close to inferior alveolar nerve,in order to reduce the postoperative complications of tooth extraction.Method:Nearly two years(2013.12 ~ 2015.12)this study selected 30 cases of patients who require removal of impacted mandibular third molars from affiliated stomatological hospital of nanchang university,14 cases of male,16 cases of female,20 to 35 years old,the average age is 24.6±3.6 years old.The total impacted teeth are36 cases,16 cases are on the left side,20 cases are on the right side,the horizontal and mesioangular impacted mandibular third molars both are 18 cases.As the research object,are included in the standard which is mainly by the cone beam CT scan that roots are close contact to the alveolar nerve canal.36 impacted teeth by a new microimplant anchorage traction aid pulling,when roots are away from alveolar nerve canal and then pull out the left teeth.Observe the needed time for pulling roots away from the alveolar nerve canal,root traction distance,teeth loose degrees after pulling,the time needed for tooth extraction,the complications after tooth extraction.Using spss19.0 for data processing,traction,tooth extraction time and the influence factors of residual tooth tissues for mobile distance with t test or analysis of the single factor variance analysis;Loose degree of influence factors is analyzed in the contingency table test,n < 5,using chi-square fisher's exact probability method,P ?0.05 or less difference was statistically significant.Result:36 mandibular third molars by microimplant anchorage traction method after 1 ~3 weeks(average time was 1.86±0.789 weeks),average root mobile distance was2.97±0.22 mm,the remaining tooth loosening occurred I ~ III°,the average extraction time was 2.19 min.Patients had no inferior alveolar nerve injury and no obvious discomfort after tooth extraction.By the single factor analysis root morphology,root tip type and maxillary depth and traction root by the speed with the left mandibular neural tube;Type of root tip and maxillary depth and traction movement distance how much matter,Root morphology and maxillary depth and size after pulling teeth loose degree has relations;Root morphology,root tip type and maxillary depth and time needed for the last tooth extraction.ConclusionsApplication of a new microimplant anchorage traction method that pulling out impacted third molar which close to the alveolar nerve canal,is a reliable method of minimally invasive tooth extraction.Can greatly reduce the inferior alveolar nerve injury and severe complication of mandibular fracture,at the same time can reduce postoperative discomfort after tooth extraction.
