Objective The purpose of this research is to summarize the lateral cervical lymph node metastasis in papillary thyroid carcinoma. To analyze the risk factors of lateral cervical lymph node metastasis and the indication of neck lymph node dissection in papillary thyroid carcinoma, and guide standardized treatment to these patients.Method Published articles about papillary thyroid carcinoma were searched in Pub Med, EMBASE and MEDLINE until January 2016 to examine the risky factors of lateral lymph node metastasis. Software Rev Man5.3 was used for meta-analysis.Result The rates of lateral cervical lymph node metastasis in papillary thyroid carcinoma were ? 10.3%(87/844), ? 18.2%(154/844), ? 16.8%(142/844), and ? 2.2%(17 / 782). There have moderate intensity relationship between lateral cervical lymph node metastasis and younger than 45 years old, male, tumors larger than 10 mm, capsule invasion, extrathyroidal invasion, multifocality, bilaterality and tumor location. There have strong intensity relationship between the number of central lymph node metastasis, especially the number ?2, and lateral cervical lymph node metastasis in papillary thyroid carcinoma. It has no relationship between hashimoto's thyroiditis and lateral cervical lymph node metastasis in papillary thyroid carcinoma.Conclusions The most common compartment of papillary thyroid carcinoma lateral cervical lymph node metastasis is ? compartment, then followed ?,? and ?. ? compartment rarely occur in lateral cervical lymph node metastasis. Lateral cervical lymph node metastasis relates with patients' age, sex, tumor size, capsular invasion, extrathyroidal extension, mulifocality, tumor location. The central lymph node metastasis is high risk factor for lateral cervical lymph node metastasis.
