Objective : To investigate the clinical pathological features of multifocal papillary thyroid carcinoma and the features of neck central and lateral lymph node metastasis.Methods:Collect the clinical pathological data of 187 patients who had a operation of PTC and divide these patients into two groups,one is multifocal and another is solitary focal.Contrast and analyze the clinical data to summarize the characteristic in neck central and lateral lymph node metastasis of the two groups.Results:There was no significant difference between the multifocal group and solitary focal group in gender,age,tumor diameter,and Hashimoto's thyroiditis(P >0.05),but the multifocal group is more susceptible to neck central lymph node metastasis(P<0.05)and neck lateral lymph node metastasis(P<0.05);in the multifocal group,the patients of tumor numbers =3 or more than 3 had a higher rate in neck lateral lymph node metastasis(P<0.05),and had no significant difference in neck central lymph node metastasis(P>0.05)compared with cases with 2 lesions;the bilateral multifocal group have a higher rate in neck lateral lymph node metastasis(P<0.05),and had no significant difference in neck central lymph node metastasis(P>0.05)compared with the unilateral multifocal group;PTC patients with Hashimoto's thyroiditis were more prone to neck central lymph node metastasis(P<0.05)and neck lateral lymph node metastasis(P<0.05).Conclusion: Multifocal papillary thyroid carcinoma is more prone to neck central and lateral lymph node metastasis,and is more dangerous with the increase in the tumor numbers.The bilateral multifocal papillary thyroid carcinoma is more prone to lateral lymph node metastasis than unilateral multifocal papillary thyroid carcinoma.Multifocal papillary thyroid carcinoma patients with Hashimoto's thyroiditis and other high risk factors for neck lateral lymph node metastasis were suggested to be performed neck lateral lymph node dissection.
