BackgroundWith the popularity of the coronary angiography, more and more coronary artery diseases phenomenons were found, slow coronary flow phenomenon is one of them. Slow coronary flow phenomenon is coronary artery without obvious stenosis refers to the contrast agent perfusion delay. The incidence of this phenomenon of domestic and foreign reports was different,Hawkins found 5.5% suspected of coronary heart disease in patients with SCF, others reported that approximately 7% patients had slow coronary flow phenomenon when patients were performed coronary angiography. At present,the pathogenesis of slow coronary flow phenomenon is closely related to the coronary vascular endothelial dysfunction, blood viscosity, coronary artery disease, microvascular disease and so on, it may lead to myocardial ischemia or severe arrhythmia events, etc. many researches concerned about the blood biochemical and vascular endothelial dysfunction,but paid less attention to the neural regulation disorder at present.Holter is One of the important and common auxiliary examinations in Cardiovascular department, it not only can show the presence of all kinds of arrhythmia phenomenon,but also can effectively reflect the patient's heart rate variability, we can evalue tension and balance of the cardiac vagal and sympathetic nerve activity and its effects on the cardiovascular activities. PNN50 is an important indicator of heart rate variability in the holte, it is the percentage of the difference of adjacent sinus R-R interval over 50 ms in all adjacent sinus R-R interval,which mainly reflects the level of vagal nerve tension. In this study, we evaluated the change of nerve tension in patients with slow coronary flow phenomenon by means of this test, and analyzed the correlation between the change of the PNN50 and coronary slow flow phenomenon. ObjectiveTo investigate the correlation between PNN50 and slow coronary flow phenomenon. MethodsConsecutive selecting patients from January of 2014 to February of 2016 Department of Cardiology in the First Affiliated Hospital of Zhengzhou University, CAG showed no significant stenosis of the coronary artery and tumor like dilatation of 300 patients. recording the relevant clinical data,dividing the patients into two groups according to the TIMI frame count,the slow coronary flow group(n=123,57 males, age54.7±9.02) and the normal blood flow group(n = 177,86 males,age53.62±8.91). Two groups of patients were performed 24 hour holtor,after analysising the heart rate variability index of two groups,we compared the difference in PNN50 between the two groups, Logistic regression analysis was used to analyze the influencing factors of slow coronary flow phenomenon. ResultsThe hematocrit level rate and the level of C reactive protein in the SCF group was higher than that in the normal group,with significant difference(39.7±3.8 vs.37.3±3.3 P<0.05 and 12.35±4.73 vs. 9.66±3.20 P<0.05),PNN50 was lower than that in the normal group(14.2±7.3 vs. 22.8.±6.2,P<0.05). The results of Logistic analysis showed that the red blood cell volume and C reactive protein were the risk factors of slow coronary flow, and PNN50 was the protective factor of the slow flow of coronary artery. ConclusionThe decrease of PNN50 is an independent risk factor of the slow coronary flow phenomenon.
