Background andCerebral vascular disease is a serious threat to the human health,with the improvement of human's living standard,people significantly improve their concern to the health,and the needs for early diagnosis and early treatment of disease is increasing urgent.Clinical studies have found that there is a close relationship between cerebral vascular disease and the hemodynamic abnormal changes in cerebral circulation system,and the shape of the cerebral vascular directly affects the hemodynamic changes.Vertebral basilar artery is the most important blood vessel in posterior cranial fossa and is an important part of the posterior circulation of the brain.It is mainly composed of the intracranial segment of vertebral artery,basilar artery and its main branches.The trunk and branches' blood supply range is large,and its function is widely distributed,which mainly supply the important structure such as brainstem,cerebellum,temporal below the leaf,pillow sides of the leaves,and its morphological structures have the important significance in the anatomy and clinical application.Cerebellar atrophy is a kind of manifestation of neurological imaging in clinic,which often occurs in the middle and late stage.Many scholars have made a lot of researches on the cerebellar atrophy's mechanism,clinical manifestation,imaging characteristics and treatment.There was no report about whether the morphological changes in the vertebral basilar artery and the changes of hemodynamics is connected with the occurrence of cerebellar atrophy.In recent years,the continuous development of imaging technology,especially the rapid development of multilayer spiral computed tomography(MSCT)and the computer technology,which make the CT angiography(CTA)widely used in clinical practice.With its advantages such as safety,quickness and non traumatic,multislice computed tomography angiography(MSCTA)has gradually replaced the "gold standard" of the invasive digital subtraction angiography(DSA)examination.Currently MSCTA has been widely used in the examination of vascular system through the whole body,especially in the head and neck CTA,coronary artery CTA and lower extremity arterial CTA application.The purpose of this study is to observe the correlation between the morphological abnormalities of the intracranial vertebral basilar artery system and the atrophy of the cerebellar cortex by CTA of the head and neck,and provide references for clinical diagnosis and the treatment of related diseases.MethodSelect 225 cases CT angiography in head and neck of Liaoyang Third People's Hospital in 2010~2014,including 117 males and 108 females,aged 36-78 years old,with an average of 57.35 ± 10.35 years old.Of which,Cerebellar atrophy 123 cases,age 37~78 years old,with an average of 59.00± 10.08 years old;non cerebellar atrophy 102 cases,age 36~77 years old,average 54.50 ± 10.27 years old.Three dimensional images of the intracranial vertebral basilar artery system and its main branches were reconstructed by the SIEMENS 64 slice spiral CT,and we measure the diameter of the lumen of the intracranial vertebral basilar artery and observe its morphology and make the statistical analysis.Results1.In the the cerebellar atrophy group,the left vertebral artery normal is 26 cases,tortuosity is 77 cases,hypoplasia is 3 cases,segmental occlusion or stenosis is 3 cases,the absence is 14 cases.The normal incidence rate was lower than that of the normal comparison group,the tortuosity and absence of the incidence rate is higher than the normal comparison group,and the difference has statistical significance(p<0.05).2.In the the cerebellar atrophy group,the right vertebral artery normal is 34 cases,tortuosity is 66 cases,hypoplasia is 9 cases,segmental occlusion or stenosis is 5cases,the absence is 9cases.The normal incidence rate was lower than that of the comparison group,the tortuosity incidence rate is higher than the normal comparison group,and the difference was highly significant(p<0.01).3.In the the cerebellar atrophy group,the basilar artery normal is 25 cases,tortuosity is 93 cases,segmental occlusion or stenosis is 3 cases,the variation of open window is 2 cases.The normal incidence rate was lower than that of the normal comparison group,the tortuosity incidence rate is higher than the normal comparison group,the difference was highly significant(p<0.01).4.The diameter of the left and right vertebral artery and basilar artery in the cerebellar atrophy group were 2.64±1.31 mm,2.26±1.04 mm and 3.33±0.65 mm,and were thinner than those in the normal comparison group,and the difference was significant(P<0.05).5.The diameter of the left and right posterior inferior cerebellar artery in the cerebellar atrophy group is 1.36±0.50 mm and 1.21±0.58mm;the diameter of the left and right anterior inferior cerebellar artery is 1.26±0.55 mm and1.30±0.54mm;the diameter of the left and right superior cerebellar artery is 1.74±0.29 mm and 1.70±0.34 mm.And they were all smaller than the value of the normal comparison group,the difference of posterior inferior cerebellar artery and inferior anterior inferior cerebellar artery was highly significant(p<0.01),but the superior cerebellar artery's difference has no statistical significance(p>0.05).6.There was no significant difference in the age and sex of vertebral basilar artery abnormality(p>0.05).Conclusion1.CTA examination can be simple,rapid,noninvasive,complete and intuitive to show the morphology of the vertebral basilar artery,and can find a variety of developmental and anatomical abnormalities.2.The tortuosity of vertebral basilar artery morphology may be related to the onset of cerebellar atrophy.3.The diameter of the vertebral basilar artery and its major branches of the cerebellar atrophy may be thin,which may result in the insufficiency of the blood supply of the cerebellum.Vertebral-basilar artery tortuosity can directly influence the diameter of posterior inferior cerebellar artery and the inferior anterior inferior cerebellar artery,but it has little effect on the diameter of the superior cerebellar artery.4.There was no significant correlation between the morphology of the vertebral basilar artery and the age and sex.
