In recent years,lung cancer has become a serious hazard to human health.The mortality of lung cancer were increased significantly in the world and lung cancer has been the first cause of tumor death for many years in China.Prevention and treatment of lung cancer have become one of the important public health problems.Some epidemiological studies have reported several common risk factors of lung cancer,including smoking history,respiratory disease history,susceptibility genes.Our research explored the strength of relation of some risk factors(such as lifestyle,mental and psychological factors,occupational environmental factors,diet,past history of respiratory disease and family history of cancer)of lung cancer through a case-control study in southern Chinese and meta-analysis on worldwide studies for providing evidence of cause of lung cancer and theoretical basis for the prevention and control of lung cancer.Part 1.A Case-control Study on the Risk Factors of Lung CancerObjective: To explore the influence of several common risk factors of primary lung cancer,we hope to provide the basis cause of lung cancer.Methods: A case-control study was carried out.The cases are the patients with the primary lung cancer diagnosed in Beijing military general hospital between November2014 and December 2016.126 cases of patients with lung cancer and 118 controls were interviewed about the exposure information of related factors using the questionnaire.The questionnaire interviewed included lifestyle,mental and psychological factors,occupational environmental factors,diet,past history of respiratory disease and family history of cancer and then all analyses were performed using the Statistical Package for the Social Sciences software version 22.0 for Windows.Results: There is a significant increase in the risk of long-term smoking and passive smoke exposure in patients with lung cancer,the OR values were 1.341 and 1.196;inappropriate drinking behavior and the long-term lack of sleep increased 1.828 and2.04 times at risk of lung cancer;People with inharmonious marriage,life stress and long-term mental depression is susceptible to lung cancer,poor emotional selfregulation ability or negative living attitude can also increase the risk of lung cancer;Exposure to asbestos,benzene,metal dust,coal dust and fume fuel increase the incidence of lung cancer;Eating more fresh fruits,vegetables,fish,and more garlic are the protective factors of lung cancer,but eating too much pickled food will increase3.073 times of lung cancer risk;There was statistically related between pulmonary tuberculosis history,history of COPD lung disease and lung cancer,and people with family history of cancer increased 10.049 times of lung cancer risk.Conclusions: Long-term heavy smoking,passive smoking,long-term lack of sleep,pessimism,long-term mental depression,exposure to asbestos,metal dust,fumes from the kitchen fuel,edible pickled food and chronic respiratory disease history(such as pulmonary tuberculosis,chronic obstructive pulmonary disease(COPD)disease)increase the risk of lung cancer,so we should find special groups and take effective measures to prevent and treat lung cancer.Part 2.XPD Gene Polymorphism and Lung Cancer Susceptibility:A Meta-analysisObjective: Xeroderma pigmentosum group D(XPD)gene plays an important role in the process of nucleotide excision repair(NER).Genetic variations in the single nucleotide polymorphism of XPD gene may increase cancer susceptibility.Lys751 Gln and Asp312 Asn are two common polymorphic site of XPD gene.We performed a systematic review and conducted a meta-analysis to explore association of the XPD Lys751 Gln and Asp312 Asn polymorphism with risk of cancer.Methods: We searched through Pub Med,Embase,Medline and China National Knowledge Infrastructure for all eligible studies published up to March 1st 2016.With criteria for inclusion and exclusion,case control study was screened out about the association between ERCC2/XPD Lys751 Gln and Asp312 Asn and lung cancer.By the aid of STATA13.0 software,the pooled OR and 95%CI were calculated,heterogeneity among combined studies was assessed.At the same time,we conduct subgroup analysis by different race,different source of control and different sample size.Results: Until March 1st 2016,we found 34 studies from 32 Publications about the relation to Asp312 Asn and Lys751 Gln Polymorphisms(19542 cases and 25078controls).Genotype Asn/Asn,Gln/Gln and Lys/Gln increase lung cancer susceptibility(p<0.05).Conclusions: ERCC2/XPD Asp312 Asnand Lys751Gln are connected with lung cancer susceptibility as independent risk factors,especially Asn allele in Asians,751Gln/Gln and 751Lys/Gln in Caucasians,Asians,population-based and hospital-based groups increase lung cancer risks.
