Objectives:To discuss the relationship between blood glucose changes and body's immune function in patient with acute pancreatitis.Methods:A total of 63 patients with acute pancreatitis who were treated in our hospital were enrolled in this study from March 2014 to November 2015.The inclusion criteria were:any age and gender;without the history of diabetes;without any clinical symptoms of diabetes;without the history of using hormone drugs recently;not taking hypoglycemic drugs before hospital admission;no merged thyroid,adrenal gland functional disease and other diseases affecting the body's glucose metabolism.The patient's venous blood glucose were detected on admission and the second day early in the morning,and then taken the mean respectively?Sixty-three patients were allocated into 3 groups according to the mean of blood glucose: Blood glucose < 7.2mmol/L(group A,n=9);Blood glucose :7.2 —11.1mmol/L(group B,n=31);Blood glucose>11.1mmol/ L(group C,n=23)?Blood samples were collected before treatment and on the 2nd,4th and 6thday after conventional treatment respectively to measure the serum levels of cytokines(TNF-??IFN-??IL-6?IL-2),the percentages of T lymphocyte subsets(CD3+?CD4+?CD8+?CD4+/CD8+)and the percentages of NK cells.The changes of parameters of the three groups at different time point were compared respectively?Results In group C,the percentages of CD3+,CD4+,CD4+/CD8+ and NK cells and the serum level of IL-2 were significantly lower than those in group A(p<0.05);the percentages of CD4+,CD4+/CD8+ and the serum level of IL-2 were remarkably lower than those in group B(p<0.05),while the serum levels of TNF-?,IL-6 were obviously higher than the other two groups(p<0.05)?In group B,the percentages of CD3+ and NKcells and the serum level of IL-2 were significantly lower than those in group A(p<0.05),while the serum level of IL-6 was higher than that in group A(p<0.05)?After treatment,the serum levels of IL-2 in group B and C had an upward trend,while the serum levels of IL-6,TNF-?both showed a trend of descending?In group B,the serum level of IL-2 on the 2nd day after conventional treatment was visibly higher than that before treatment(P<0.05);on the 4th day after treatment,the serum levels of TNF-?and IL-6 were obviously lower than those before treatment(P<0.05);on the 6th day after treatment,the percentage of CD3+ and the ratio of CD4+/CD8+ were significantly higher than those before treatment(P<0.05)?In group B,the serum level of IL-2 on the 2nd day after conventional treatment was visibly higher than that before treatment(P<0.05);on the 4th day after treatment,the serum levels of TNF-?and IL-6were obviously lower than those before treatment(P<0.05);on the 6th day after treatment,the percentage of CD3+ and the ratio of CD4+/CD8+ were significantly higher than those before treatment(P<0.05)?Conclusions : The hyperglycemia of patients with acute pancreatitis resulted in disorder of body's immune function;The higher the level of blood glucose was,the more damage of the body's immune function.
