The etiology and the pathogenesis of the Chronic Idiopathic Urticaria(CIU) are very complicated, and now the neuroendocrine-immune system dysfunction is thought to be related to the onset of CIU. The objectives of the study are to investigate the changes of DHEA-S concentration on the CIU patients and explore the relations between the serum of DHEA-S in the CIU patients and the gender, age, course, autologous serum skin test(ASST), and the potential correlation between the imbalance of Th1/Th2 cells and the serum of DHEA-S in the CIU patients, and observe the changes in endocrine and immune state of the CIU female patients who received the anti-histamine treatment. With a random selection, seventyseven CIU patients, who were operated in the Department of Dermatology,the First Affiliated Hospital of Hebei North University, were divided into three groups. The first was divided into male and female groups according to the gender; the second was divided into ASST positive and ASST negative groups according to the results of autologous serum skin test; the third, the CIU female patients were divided into childbearing age group,perimenopausal and postmenopausal groups according to the menopausal.And all the CIU female patients of the three groups received the treatment of the loratadine and chlorpheniramine with the oral antiallergics. Among the seventy-seven experimental subjects, fifty-seven in healthy control subjects, nineteen natural menopause in postmenopausal control groups.The ELISA method was used to examine the level of the DHEA-s, IL2, IL4 and IFN-?. The SPSS17.0 statistical software applied to data entry and statistical analysis.The results shown that:(1) the level of the DHEA-S in CIU group was lower than the healthy control group(P<0.05); the level of the IL2 and the IFN-? were both lower than the healthy control group(P<0.05).(2)regardless of the results of the autologous serum skin test, the DHEA-S level of the ASST positive group and ASST negative group were both lower than the healthy control group(P<0.05).(3) the level of the DHEA-S in CIU female group(P=0.01) was decreased, and much lower than that in the CIU male group(P=0.028); the level of the DHEA-S and the IL-4between the CIU female postmenopausal group and postmenopausal control group are similar(P>0.05), but the level of the IL-2 and IFN-? in the CIU female were both lower than the postmenopausal control group(P<0.05)(3) the level of the serum of DHEA-S in the CIU patients had a positive correlation with the level of the IL-2(rs=0.235) and the IFN-?(rs=0.041), and had a negative correlation with the level of the IL-4(rs=-0.290)(4) the course of the disease had no obvious correlation with the DHEA- S, and IL-2, IL-4, IFN-?(rs=0.146, 0.087, 0.036,-0.030).(5)among three CIU female patients groups who received the antiallergics treatment, the effective rate of the treatment in perimenopausal group was lower than the childbearing age group and postmenopausal group,but the results were not statistically significant(P>0.05); the level of the DHEA-S in the positive CIU female group was lower than the negative group and the control group, and the results were statistically significant(P<0.05); the level of the DHEA-S in the negative CIU female group had no significant difference with the control group(P>0.05).The above results suggest that: the reduced level of the DHEA-S in the peripheral blood has a potential correlation with the imbalance of Th1/Th2 cells and its clinical state. Probably the exaltation level of the DHEA-S in the CIU patients is conducive to recuperating the imbalance of Th1/Th2 cells.(2) the CIU female patients has various performances on the level of the DHEA-S?IL-4?IL-2 and IFN-? because of their different onset ages and clinical states, which will provide a new direction to study the pathogenesis of the CIU and seek more effective treatments.
