Objective: Chronic renal failure has become one of the main lethal factors of diabetes. In this study,hospitalized diabetic patients are randomly selected as the research object,to investigate the relationship between glycosylated hemoglobin(Hb A1c) and creatinine clearance rate in patients with early type 2 diabetic nephropathy.Method:190 hospitalized patients with type 2 diabetes mellitus are collected,all in line with the DM diagnosis standards promulgated by the WHO in 1999,not having history of hypertension or blood pressure is under control,not having acute or chronic nephritis,heart,brain,lung and liver disease,and not affected by factors such as infection, fever, diabetic foot etc.Renal function installment is according to the K/DOQI. Patients with stage 1-2 of the renal function are selected as the research object, a total of 126 cases, divided into two groups according to the different Hb A1c: group A for Hb A1 c of 6.5% or less, 65 cases, mean age57.54±12.99 years old; group B for Hb A1c> 6.5% group, 61 cases, mean age 53.48±12.77 years old. Immune turbidimetric method is used to detect glycosylated hemoglobin. Enzymic method is used to determinateserum creatinine(Scr).The MDRD formula is used to calculate glomerular filtration rate.The Cockcroft-Gault formula is used to calculate creatinine clearance.SPSS13.0 statistical software is used for statistical analysis,with counting data by chi-square test, and measurement data with mean±standard deviation.Analysis of variance is used for comparison between the two groups of measuring data using,with variance by t test, and heteroscedasticity by t ? test.Correlation analysis is used in the case of no grouping. The difference is statistically significant when P<0.05.Results: 1?IN comparison with the two groups of patients' renal function in installment distribution,gender,age and body mass index, there is no statistically significant difference(P > 0.05). 2?Glomerular filtration rate and creatinine clearance value in B group are higher than of group A(P < 0.05), with statistical significance. 3?In the case of no grouping on correlation analysis of creatinine clearance and glycosylated hemoglobin,diabetes duration, glomerular filtration rate,there is a positive correlation(P < 0.05).Conclusion:In type 2 diabetic patients with early nephropathy, the higher the Hb A1 c, the higher the glomerular filtration rate and the creatinine clearance, which reflects the more serious conditions of glomerular high perfusion and high filtration.
