| Objective : This study was conducted by statistic analysis of general information of primary hypertension patients,observes left ventricular diastolic function and left ventricular structure by the application of ultrasonic echocardiography,analyze the characteristics about the change of the left ventricular diastolic function and the influencing factors preliminary,provides a reliable indicator for the assessment of left ventricular diastolic function in patients with essential hypertension.Methods:This study selected 100 cases of patients with primary hypertension in the age of 40 to 70 years old,according to the diagnostic criteria of hypertension grade standard, divide the patients who met the inclusion criteria into three groups respectively, grade 1 hypertension group and 2 group? 3 group, 60 cases in the control group with normal blood pressure group in the age of 40 to 70 years old,the control group was divided into normal blood pressure group and high normal blood pressure group. The difference data were compared between groups of gender, age and course of disease, weight index, biochemical index etc,observe and record the evaluation indexes of left ventricular diastolic function and indexes of left ventricular structure in patients with different group by the application of ultrasonic echocardiography,then deal with data by using statistical software,analyze the correlation between evaluation index of left ventricular diastolic function and general data,index of left ventricular structure in patients of each group.Results:1. The general situation analysis:(1)Gender differences between male and female patients with grade 3 hypertension group was significant (p<0.05),body mass index of patients in grade 2 hypertension group and 3 group were significantly higher than the normal group and high normal group(p<0.05).(2)There was statistically significant in the smoking factors of patients in grade 3 hypertension compared with control group, grade 1 hypertension group(p<0.05);There was statistically significant in the drinking factors of patients in grade 3 hypertension compared with control group(p<0.05).(3)Difference is obvious in triglyceride and blood glucose between the 1, 2, 3 levels of hypertension group(p<0.05).(4)There was statistically significant about the number of patients in intimal thickening of the carotid artery between the 3 groups in hypertension(p<0.05).2. Heart function analysis:(1)Difference is obvious in Ea/Aa and E/Ea between groups of hypertension in multiple comparison(p<0.01),grade 3 hypertension group is more obvious than grade 1 and 2 groups.(2)There was statistically significant in the index EF of patients in grade 3 hypertension compared with the other groups,contraction index EF reduces obviously only in grade 3 hypertension group.(3)The level of LVDd in the grade 3 hypertension was significantly higher than that in the other four groups(p<0.05),difference is obvious in LVMI between the grade 1, 2, and 3 groups of hypertension. Thickening is obvious in IVST and LVPWD between grade 2 and 3 hypertension group compared with the other three groups( p < 0.05).3. Correlation analysis:(1)There is negative correlation between Ea/Aa and age?BMI?smoking history?course of disease?LVDd?LVMI,there was statistically significant(p<0.05),there is positive correlation between E/Ea and BMI?smoking history?LVDd?LVMI,there was statistically significant(p<0.05).(2) Ea/Aa and E/Ea all have maximum correlation with LVDd.Conclusion : 1.Male were significantly larger than females in grade 3 hypertension group, smoking and drinking numbers had significant difference in grade 3 hypertension group compared with the rest of the groups, blood glucose ? triglyceride ? carotid artery intimal thickening number have significant difference when have an multiple comparison in hypertension groups, can be regarded as one of the evaluation indexes of the severity of hypertension.2.Decline of diastolic function is far earlier than systolic dysfunction about patients with essential hypertension,diastolic function decreased in all hypertension groups,grade 3 hypertension group decreased significantly. Grade 3 hypertension group LVEF compared with other groups, there was significant difference in systolic function decreased significantly.Compared with the other groups, there were significant differences in the LVEF in the grade 3 hypertension group, contractile function decreased significantly in the grade 3.3.Ea/Aa and E/Ea is the more accurate evaluation index of left ventricular diastolic function in essential hypertension patients.4.Left ventricular diastolic function of patients with essential hypertension are closely related to age?BMI?smoking history? course of disease?LADd and LVMI, LADd have the largest correlation among them,LADd is also a good indicator of left ventricular diastolic function. |
