Risk Factors For Predicting Early Variceal Rebleeding After Endoscopic Variceal Ligation

Objective: To investigate the clinical risk factors in early variceal rebleeding after endoscopic variceal ligation(EVL).Materials and Methods: A total number of 171 cirrhotic patients with esophageal varices who received regular or emergent EVL to precaution hemorrhage or rebleeding at our endoscopy center during January 2012 to January 2016 were contained in this research. A case-control study was conducted contrasting the patients arising with early rebleeding with patients who didn't occur early rebleeding. The risk factors for analysis included the sex, age, aetiology of cirrhosis, Child-Pugh status, comorbidities,splenectomy history, portal vein thrombosis, general laboratory data,medical imaging,and endoscope. Thirty-nine potential factors were analyzed by t-test, chi-square test,rank sum test and univariate Logistic regression analysis to identify the risk factors of early variceal rebleeding after endoscopic variceal ligation.Results: The occurrence rate of early rebleeding after EVL was 13.45%. Stepwise multivariate Logistic regression analysis illustrated that four variables were independent risk factors for early rebleeding: first set of ligation [odds ratio(OR) 3.900,95%CI: 1.579-9.635, P<0.05],fasting plasma glucose(OR 1.696, 95%CI: 1.012-2.845,P<0.05),amount of blood transfusion before EVL(OR 8.118, 95%CI: 2.352-28.014,P<0.05) and number of varicose veins(OR 2.794, 95%CI: 1.143-6.831, P< 0.05).Conclusion: First set of ligation, fasting plasma glucose, amount of blood transfusion before EVL and number of varicose vein are independent risk factors of predicting early variceal rebleeding after EVL.
