[Objective] Neurometer quantitates the current perception threshold of sensory nerve fibers,and compare the result with nerve conduction velocity.To analyze the two methods in physical and biochemical index. To evaluate the clinical value of current perception threshold test for diagnosing type 2 diabetic peripheral neuropathy.[ Methods ] 112 patients with type 2 diabetes mellitus were enrolled in the study,Both the current perception threshold and nerve conduction velocity testing were applied in these patients. The patients were divided into four groups according to the results.The medical history, physical examination, MNSI score, OGTT and biochemical index. To analyze the difference between current perception threshold and nerve conduction velocity in the diagnosis of type 2 diabetic peripheral neuropathy. Compare the clinical information on each groups.To discuss the clinical value of current perception threshold test for the diagnosis of type 2 diabetic peripheral neuropathy.[Result](1) The patients number of NCV-CPT positive group, NCV positive group,CPT positive group and NCV-CPT negative group is 36, 8,22,46 respectively.(2)The positive rate of CPT test(51.79%) is higher than NCV test(39.26%) in the diagnosis of diabetic peripheral neuropathy(P=0.016,Kappa=0.468).(3)The positive rate of NCV test in hepersensitive subgroup of CPT is 43.75%,while in hyposensitive subgroup of CPT the rate is 84.61%. Through this result,we can conclude that CPT examination is better than NCV test in the early diagnosis of diabetic peripheral neuropathy.(4)There were no significant differences in age,gender,waist circumference,TC,HDL-C,UA,OGTT 0min blood glucose,insulin,C-peptide,HOMA-IR between the four groups(The P values were 0.662,0.260,0.828,0.651,0.174,0.209,0.420,0.791,0.333).The differences in the course,BMI,MNSI,Hb A1 c,TG,LDL-C,OGTT120 min blood glucose,insulin,HOMA-? were statistically significant between the four groups(The P values were 0.001,0.001,0.008,0.004,0.001,0.000,0.001,0,026,0.018)[ Conclusion ] Through detecting current perception threshold of sensory nerve,Neurometer has great value in diagnosing early diabetic peripheral neuropathy,and its sensitivity is higher than nerve conduction velocity test.The risk factors of diabetic peripheral neuropathy include course, BMI, Hb A1 c, OGTT2-hour blood glucose,insulin, triglyceride and low density lipoprotein.
