The Influence Of Yinchenhao Decoction Treatment For Blood Flow Dynamics And The Blood Coagulation Function In Obstructive Jaundice Patients

Objective:To observe and analysis of hemodynamics, blood coagulation function and the change of liver function in patients with obstructive jaundice. To discuss the influences of Yinchenhao Decoction on the mechanism and provide the basic theory for reasonable administration in perioperative.Methods:During June 2012- June 2014 in our hospital, 76 cases of patients with obstructive jaundice, in which there are 37 cases of choledocholithiasis, 2 cases of bile duct injury,2 cases of ampullary papillary adenoma, 8 cases of cholangiocarcinoma, 5 cases of pancreatic head carcinoma, 9 cases of icteric type hepato-cellular carcinoma and 13 cases of ampullary carcinoma. The patients accepted the surgery or endoscopic biliary drainage respectively. All of the patients without clotting mechanism obstacle, did not take the medicine of anticoagulation, antiplatelet aggregation and nonsteroidal, which have an effect on coagulation, while they denied the history of drug allergy. Then they were accepted postoperative routine therapy(control group, 38 cases) or routine treatment plus Yinchenhao Decoction(study group, 38 cases) by randomized 1:1 after adopt corresponding surgical treatment. The changes of hemodynamic index(whole blood viscosity, plasma viscosity, red blood cells deposited, blood sedimentation),blood coagulation function index(activated partial thromboplastin time(APTT),prothrombin time(PT), fibrinogen and D-dimer), changes of liver function(glutamic-pyruvic transaminase(ALT),totalbile acid(TBA),alkaline phosphatase(ALP),glutamine transferase(GGT), serum total bilirubin(TBIL), direct bilirubin(DBIL))were compared in two groups 7 days after adminstration.Results:1. Comparison between two groups of hemodynamic indexesAfter 8d of the treatment, indexes in the study group of whole blood viscosity, plasma viscosity, erythrocyte deposited have significantly reduced comparing preoperative and the control group at the same time point(P < 0.05). While blood sedimentation compared with this group of preoperative show significantly increased(P < 0.05).2. Comparison between two groups of coagulation index The indexes in the study group of PT, APTT have significantly decreased(P < 0.05)while Fib levels, blood sedimentation have significantly increased comparing with preoperative and the control group at the same time point(P < 0.05).3. Comparison between two groups of liver function After 8d of the treatment, indexes in the study group of ALT?AST?TBIL?DBIL?TBA?ALP?GGT have significantly reduced comparing preoperative and the control group at the same time point(P < 0.05).Conclusion:1. After combination treatment of routine therapy and Yinchenhao Decoction, the whole blood viscosity, plasma viscosity, erythrocyte deposited, PT and APTT, are decreased in postoperative obstructive jaundice patients.2. FIB levels, blood sedimentation have significantly increased comparing with preoperative and the control group at the same time point.3. Yinchenhao Decoction is conducive to improve the high condensation in postoperative obstructive jaundice patients, to maintain good blood circulation.Therefore, it could be recommended as routine therapy in clinical application of postoperative obstructive jaundice patients.4. The patients with obstructive jaundice were administrated by capillaris decoction after relieving obstruction through operation. The results suggested that capillaries decoction could accelerate the jaundice resolution and promote the recovery of liver function of the paients.
