| Objective To conclusion the clinical characteristics and early diagnosis methods and discusses the factors affecting the prognosis of patients with carcinoma of pancreatic body and tail,further evaluation treatment efficacy of carcinoma of pancreatic body and tail.Methods It’s a retrospective study. Analysis related cases data of 45 patients with pancreatic body/tail cancer in Ningxia Medical University General Hospital in March 2002 to March 2015,to summarize the clinical characteristics of the patients, medical history characteristics.Analysis of age, abdominal symptoms, CA19-9 value, operation mode, TNM stage and adjuvant chemotherapy on the prognosis of patients with pancreatic body/tail cancer.Results Group 45 cases, mean age 61.2 ±9.8 years of age, the sex ratio of 2.75:1(let), first symptom to the left, upper abdominal pain 29 cases(64.4%), low back pain(8.9%), 4 cases of abdominal distension in 7 cases(15.6%), and muscle weakness in 1 case(2.2%), 1 abdominal mass(2.2%) and asymptomatic four people(8.9%), Positive of CA19-9 35 cases, the positive rate of 83.3%, abdominal ultrasound were 68.9%, abdominal CT positive 45 cases, positive rate was 97.8%. Laparotomy was line 15 cases, palliative surgery 8 cases, 22 cases were radical excision, radical resection rate was 48.9%, 10 cases of poorly differentiated adenocarcinoma, 22.3%(10/45), moderately differentiated adenocarcinoma of 17 cases, 37.8%(17/45), 13 cases of well-differentiated adenocarcinomas, accounted for 28.9%(13/45), 5 cases are not clearly differentiated adenocarcinoma., TMN staging stage Ⅰ6 cases(13.3%), 12 cases(26.7%)phase Ⅱ,Ⅲ period of 17 cases(37.8%), 10 cases of stageⅣ(22.2%), postoperative chemotherapy treatment of 14 cases, By Kaplan-Meier univariate analysis showed that abdominal pain, CA19-9 increased(≥120ku/L) had no statistical significance(P > 0.05) on the prognosis of patients with pancreatic body/ tail carcinoma;age, TNM stage, surgery, adjuvant chemotherapy has statistical significance(P < 0.05). Further COX regression analysis of multiple factors, the operation method is independent impact factors affecting body tail of pancreas cancer prognosis.Conclusion The pancreatic body/tail carcinoma occurs in older men, early symptoms of lack of specific clinical manifestations;detection of CT and CEA combined with CA19-9 in the abdomen is of great significance to the early diagnosis of pancreatic body/tail carcinoma. Operation method is an independent factors affecting the prognosis;active radical treatment will significantly improve the prognosis of patients. Improve the early diagnosis and radical resection is the important measures to improve the patients quality of life and survival in present. |
