Objective:1. Study on the correlations among platelet count, D-D, fibrinogen and tumor markers CEA, CYFRA21-1, NSE and non small cell lung cancer stage;2. Screening and detection of prognostic indicators for non small cell lung cancer;Methods:1. From January 2008 to January 2013, we selected 400 patients with non small cell lung cancer who underwent thoracic surgery lobectomy resection and pneumonectomy, including 265 cases of male patients and 135 female patients from Tianjin chest hospital. Each patient underwent a series of preoperative examinations, including the chest, upper abdomen CT, skull scan, line CT or MRI, and no marked signs of metastasis, except for some patients undergoing pet-CT and whole body bone scan. Patients whose cardiac function and lung function do not meet the requirements of surgery were excluded. Patients taking oral aspirin should stop for more than 1 week. Patients need to sign informed consent. Sixty-six men and 34 women, all together 100 subjects, were randomly selected from the health examination center of the hospital, as the control group.2. Early in the morning, fasting venous blood samples were taken to measure plasma PLT level, FIB level and D-D level before operation. The normal value of PLT was between 100-300 * 109/L. Fib normal value 2.0-4.0g/L,D-D normal value below 0.5mg/L. Serum tumor markers CEA, CYFRA21-1 and NSE levels in fasting venous blood were determined before operation. The normal value of CEA was between 0-4.7 ng/L. The normal value of CYFRA21-1, 0.1-3.3 ng/L. The normal value of NSE I, 0-16.3ng/L. Statistical software, SAS 9.2 version, was used to carry out data collection and analysis.Results:1.Results of Single factor ANOVA showed that average ages had significant differences among groups in different TNM stages. By LSD comparison, age differences among health group and stage IA, stage IA and IIIA staging were statistical significance.Chi square test showed that there was no significant difference in the distribution of sex among different groups. Kruskall-Wallis test results showed that the FIB, PLT, CEA, CA21-1 and NSE in different TNM stages had significant differences(P<0.01).2.Bonferroni comparison showed that levels of FIB, PLT, CEA, CA21-1 were significantly different between health group and non small Cell Lung Cancer in all TNM stages; levels of FIB, CEA, CA21-1, NSE have significant differences between patients in stage IA and stage IIIA. Levels of CA21-1 and NSE had significant difference between IA stage patients IB stage patients and IIIA stage patients.Conclusion:1. Test indicators of venous blood such as FIB, PLT, CEA, and CA21-1 for patients with non small cell lung cancer have significant correlations with TNM stages. And these indicators are higher for higher levels of non small cell lung cancer.2.In the NSCLC subgroup, FIB and CA21-1 were positively correlated with TNM stage in patients with squamous cell carcinoma. FIB and CA21-1 levels were higher in patients with advanced TNM stage. In addition, CEA, FIB, PLT and CA21-1 were positively correlated with TNM stage in non squamous cell carcinoma patients. Higher levels of these indicators were obsered in patients with advanced TNM stage.3. Therefore, our results show that FIB, PLT, CEA, CA21-1 can be used as prognostic indicators for non small cell lung cancer before surgery.
