Objective The clinical features of M were simulated and the clinical features of SFMC were decreased, and the SFMC model was constructed to determine the effect of the reduction and internal fixation on the morphology, function and histopathology of TMJ.Method 1,Position invariant to the direction to the midpoint of the lower edge of the long axis parts of hyperextension of the building on the right side of the condylar SFMC animal model: general anesthesia, regular disinfection towel; by before tragus incision, cut the skin, subcutaneous tissue flap; exposure the TMJ articular capsule and zygomatic arch; along the condylar longitudinal incision of the joint capsule, and then along the zygomatic arch root transverse incision joint capsule, the formation of a corner shaped incision, the exposed right condyle. The condylar articular surface of the condyle neck of condyle Pterygoid Fossa below inclined to the midline longitudinal incision of condyle; condylar fracture of condyle to the passage to the articular tubercle before the bottom, keep the condylar stump, the formation of the lower jaw ramus height decreased SFMC models. 2, the mandibular ramus free fall type SFMC models prepared after, retaining condylar free end in the front lower portion of the articular tubercle, layered suture incision and conventional breeding is conservative treatment group; mandibular ramus free fall type SFMC modeling was completed after immediately and will dislocation of condyle fracture anatomical restoration, and the condylar holding forceps condylar fracture and condylar stump close contact. The lateral tension screw is fixed at the end of the condylar process through the residual end of the condylar process, and the condylar fracture plate and the condylar stump can reach the anatomical position and strong internal fixation. 3, In accordance with the postoperative 4 weeks, 8 weeks, 12 weeks of time, to the temporomandibular joint(TMJ) results and function, image data and tissue pathological methodology evaluation of condylar free end of the process of reduction and internal fixation of SFMC rigid internal fixation, fracture healing, condylar reconstruction, morphology and function of the TMJ.Results In this study, we successfully constructed a SFMC animal model of the non descending type of the sheep’s dislocation type. Through conservative treatment and surgical treatment group,at different time points, compared with the preoperative, conservative treatment group maximum opening, protrusion and left to exercise the degree of reduced significantly, surgical treatment of mandibular movement is no significant change, but occlusion no significant difference. Mean radiographic score, surgical treatment group was significantly lower than the conservative treatment group. From the pathological observation, in the treatment of 4 weeks, the condylar surface has irregular new bone formation, the conservative treatment group,the new bone formation in the treatment group was significantly higher than the surgical treatment group. At 12 weeks, conservative treatment group operated animals lateral condylar morphology changes were more obvious than before, even osteophyte formation and condyle deformity. In contrast, the morphology of the lateral condyle of the surgical treatment group tended to be normal. No changes in the morphology of the non operative lateral condyle of all animals. Conclusion 1.This study successfully established the model of SFMC with a high degree of reduction in the height of the mandible. 2.The surgical treatment in experimental animals treated with passive mouth opening, mandibular lateral movement of reachability is conservative treatment group good, is conducive to the recovery of the mandibular movement. 3.The results of CBCT examination and TMJ morphological changes of the 2 groups of experimental animals showed that the anterior and posterior diameter, internal and external diameter of all the surgical side were increased. Because the inner diameter and the diameter of the condyle in the conservative treatment group were significantly increased, and the change was more obvious with the passage of time, the change of the inner diameter and the diameter of the condyle in the experimental group was not obvious. The experimental group, the surgical treatment group was lower than the control group. Therefore, conservative treatment is more obvious than surgical treatment of condylar hyperplasia, is not conducive to the restoration of TMJ morphology and function. 4.The condylar morphology of the conservative treatment group after 12 W was abnormal, and the condylar morphology of the surgical treatment group was restored to normal after 12 w. Therefore, surgical treatment of condylar morphology is better than conservative treatment, it is beneficial to the recovery of TMJ shape and function. 5.From the point of view of tissue cytology, the conservative treatment group had more new bone formation than the surgical treatment group, condylar morphology and structure deformity was not conducive to the recovery of TMJ shape and function.
