Objective:To analyze the pattern of lymphatic metastasis and influence factor of esophageal carcinoma that invades less than adventitia, in order to explore the better choice for the operation method, and optimize the curative perscription.This research consists two sections.Section 1:The lymphatic metastasis influence factor of esophageal carcinoma invades less than adventitiaMethods:We reviewed the clinical data from 484 patients receive esophagectomy from January 2011 to August 2014, whose carcinoma invades less than adventitia. The lymph node metastasis pattern of the primary tumor and corresponding influence factor were analyzed.Results:16 of 61 upper thoracic esophageal carcinoma patients have lymphatic metastasis.55 of 201 middle thoracic esophageal carcinoma patients have lymphatic metastasis.84 of 222 lower thoracic esophageal carcinoma patients have lymphatic metastasis. The deeper tumor invades,the easier lymph node metastasis occurs, as same as the lower of the tumor differentiation and the larger of the tumor diameter.Section 2:Analysis of 113 surgical resected patient's lymph node status in TIB esophageal carcinomaMethods:We reviewed the clinical data from 113 superficial esophageal squamous cell patients received esophagectomy. The lymph node metastasis pattern of the primary tumor and corresponding influence factor were analyzed. Histopathology evaluation was reviewed to ensure the tumor diameter and the submucosal invasion thicknessResult:19 of 119 superficial esophageal squamous cell carcinoma patients had lymph node metastasis. Among those patients with lymph node metastasis, the median of the tumor invasion in submucosa was 4000?m, and the range was from 50?m to 20000?m. The tumor size and the invasion depth is a predictor of node positivity.conclusion:1 The tumor size, the invasion depth and the tumor differentiation is a predictor of node positivity.2 The lymph node metastasis can occur in the esophageal squamous cell carcinoma even with the light submucosa invasion3 The location of tumor may be meaningful to the lymph node metastasis location. Therefore the surgery choice should be made comprehensively.
