| Objective:To investigate whether metabolic difference exists between essential hypertension patients and healthy subjects, while the influence of acupuncture on plasma metabolites in EH patients,Liquid Chromatography-mass Spectrometry(LC /MS) was used to study the metabolism changes in plasma.Methods:1. Study on the metabolic changes in the plasma of patients with EH:By using the LC/MS metabonomics technology was employed to analyze the difference in the plasma metabolin between EH patients and healthy volunteers, and thus the difference metabolin has been obtained.2. Study on the metabolic changes in the plasma of patients with EH treated by acupuncture:The EH patients included were divided into acupoint meridian group and non-acupoint non-meridian group. The effect of acupuncture treatment on the content of plasma metabolin of EH patients and inter-group difference in metabolin were observed with LC/MS metabonomics technology.3.The correlation study of EH plasma metabolomic data and clinical results of acupuncture:EH patients were recruited in and 24-hour ambulatory blood pressure was evaluated.Pearson Linear Correlation Analysis was applied to screen metabolites correlated with clinical outcomes.Potential biomarkers were confirmed.Results:1. Study on the metabolic changes in the plasma of patients with EH.The metabonomics method can distinguish the EH patient group from the healthy control group.Compared with the healthy control group,content of m/z126.1030(3-methylhistidine)、530.2777(glycine)、284.2949(sphingol) were significantly increased in EH patients(P< 0.05),and m/z229.1176 was significantly lower than control group(P<0.05).2. Study on the metabolic changes in the plasma of patients with EH treated by acupuncture.(1)The clinical effect of acupoint and meridian treatment on the EH patients was better than that of non-acupoint non-meridian treatment, and it can markedly improve the 24h mean systolic pressure,24h mean diastolic pressure, daytime mean systolic pressure and daytime mean diastolic pressure of the EH patients; the reduction of blood pressure level of EH patients in non-acupoint non-meridian group treated with acupuncture treatment was not significant.(2)After the administration of acupuncture intervention to EH patients, the metabolism state before and after acupuncture intervention and between the groups administered with acupuncture can also be well distinguished from the perspective of Metabonomics.(3)Content of m/z387.1283 were significantly increased after the administration of acupuncture intervention in acupoint meridian group(P<0.05), and m/z265.1175 was significantly reduced after the administration of acupuncture intervention in non-acupoint non-meridian group (P< 0.05). Compared with the non-acupoint non-meridian group, content of m/z450.2977 were significantly increased after the administration of acupuncture intervention in acupoint meridian group(P<0.05), and m/z287.2362was significantly reduced(P<0.05).3. The correlation study of EH plasma metabolomic data and clinical results of acupuncture.(1) The correlation of difference metabolin with the patients’24h mean systolic pressure and 24h mean diastolic pressure was analyzed by Pearson linear correlation analysis, the difference in m/zl26.1030 (3-methylhistidine) was significantly negatively correlated with 24h mean systolic pressure,530.2777(glycine) 607.3059(cholic acid glucuronic acid)、284.2949(sphingol)were significantly positively correlated with 24h mean diastolic pressure.(2)It was indicated by the result of Pearson correlation analysis on the value of change in the concentration of difference metabolin in the plasma of EH patients and the value of change in blood pressure of 24h mean systolic pressure and 24h mean diastolic pressure that,the value of change in the concentration of difference metabolin in the plasma of EH patients was correlated with the clinical blood pressure to some extent, the value of change in difference of m/z 211.1441 (proline) was significantly negatively correlated with the value of change in 24h mean systolic pressure, P< 0.05,263.0896 (fatty acid:3-Carboxy-4-methyl-5-propyl-2-furan propionic acid) was significantly positively correlated with the 24h mean diastolic pressure,454.3150 (fatty acid:simvastatin hydroxy acid) was significantly negatively correlated with the 24h mean diastolic pressure.Conclusions:1. There was a significant difference in metabonomics between the EH patients and healthy subjects, compared with the healthy control group, EH patients had developed disorder of different extent in terms of amino acid metabolism and lipid metabolism.2.The acupuncture had a certain regulating effect on the plasma metabolin of EH patients, the focus of acupoint and meridian treatment was different from that of non-acupoint non-meridian treatment, and they were different in the metabolin regulated from each other. The concentration of difference metabolin in the plasma of EH patients was correlated with the blood pressure. |
