Clinical Characteristics Of Systemic Lupus Erythematosus With Lymphopenia

ObjectiveThe aim of this work is according to review the clinical symptoms,signs,infection,laboratory findings,and the differences of disease activity of SLE with Lymphopenia in order to identify the causes and significance of Lymphopenia.MethodsOur study was a retrospective case-control study,We selected 193 cases with SLE in Shangdong Province Hospital from June 2013 to December 2015.They were divided into two groups:the Lymphopenia group and the control group.The clinical data included age,gender,disease duration,clinical manifestations(neuropsychiatric symptoms,vasculitis,renal damage,muscle damage,alopecia,rash,fever,etc.),laboratory indexes including routine blood test,24 hours urine protein,autoantibodies,immunoglobulin(IgG,IgM)and complement(C3,C4)etc.We use univariate and multivariate Logistic regression analysis to more than two group the clinical features and laboratory findings.ResultsThere is no statistical differences between the two groups.The incidence rate of neuropsychiatric symptoms,pericarditis,rashes,ulcers,fever,infection,bacterial infection,Leukopenia,neutropenia,Anemia,Thrombocytopenia,Anemia with Thrombocytopenia in Lymphopenia were higher than control group.The SLEDAI score in the case group was higher than that in the control group.In the case group,Anti-double stranded DNA autoantibody,Anti-nucleosome antibody,anti SM antibody,anti histone antibody,anti U1RNP antibody level was higher,and the levels of C3 were lower;Besides,anti histone antibody and anti U1RNP antibody positive rate in the case group was higher than the control group.Multivariate Logistic regression analysis showed that rash,elevated concentrations of anti U1RNP antibody or anti U1RNP antibody positive,Leukopenia,Anemia,Thrombocytopenia,Anemia and Thrombocytopenia were risk factors of Lymphopenia.ConclusionsLymphopenia is a common hematological abnormality in systemic lupus erytlhematosus.the incidence of neuropsychiatric symptoms,skin rash,ulcers,fever was higher than normal one.Anti U1RNP antibody is a risk factor for Lymphopenia.If the SLE patients with Lymphopenia,the SLE patients with other blood system damage are more danger to emerge the Lymphopenia.The correlation between SLEDAI and Lymphopenia was better than Leukopenia,so the reduction of lymphocytes can be used as a simple and inexpensive index to reflect the activity of Lupus.The SLE Lymphopenia are associated with infection,especially bacterial infection.
