Purpose:To analyze the clinical characteristics of tracheal fungus,to increase our acknowledge of the disease,and to improve the prognosis of tracheal fungus.Methods:Retrospectively analyzed the clinical date of 12 patients diagnosed tracheal fungus in Shandong Provincial Hospital from March 2012 to March 2016.The clinical characteristics,age,sex,pathogen,diagnosis,treatment and prognosis were evaluated.Results:The median age for the 12 patients was 59(range:14-74)years old,and 5 patients were above sixty.Patients comprised 4 males and 8 females(male/female ratio:1/2).The most common type was the tracheal Aspergillus(3 cases,3/12),the others were unknown.Tracheal fungus has no characteristic manifestations,cough and expectoration were the most common clinical manifestation,followed by blood in sputum.Most of the patients had underlying diseases,such as tumor,tuberculosis,diabetes mellitus,COPD etc,those could cause immunodeficiency.Risk factors included the use of corticosteroids,antibiotics,chemoradiotherapy etc.All of the patients were diagnosed by pathological examination.Endoscopic characteristics were mostly yellow-white and dark-brown neoplasm.7 patients got the pleased clinical efficacy,2 patients died because of massive hemoptysis and 3 patients that was not to be treated didn’t develop into invasive fungal infection,which can be taken for fungal colonization.Conclusions:Tracheal fungus is very few,and have no characteristic clinical manifestations.Most of the patients have underlying diseases.For the suspected patients,more inspections are necessary.Using of rational antifungal drugs according to the actual situation of patients and disease characteristics is the key to the treatment.Some fungus such as aspergillus and mucor can invade vessels,leading to life-threatening hemoptysis.Fungus that didn’t cause invasive infection can be taken for fungal colonization,on condition that it is not treated anyway,but need the regular revisit.
