Objective:Through retrospective analysis and comprehensive observation of various indicators,the angiographic manifestations and therapeutic effects of embolization in patients with massive hemoptysis involving bronchial artery in blood supply were discussed.Methods:Retrospective analysis was performed on 82 hospitalized patients with severe hemoptysis in our hospital from December 2016 to December 2018,including 51 patients who underwent bronchial artery embolization and 31 patients who underwent internal conservative treatment.Analysis both in the improvement of clinical symptoms,length of hospital stay,Complications,differences with Recurrence rate within 1 year,At the same time,the difference in efficacy between the two groups was observed to determine whether the length of hospital stay and the incidence of complications during treatment could be reduced.Results:Among the 51 patients in the bronchial arterial embolization group,41 were cured,7 showed significant effect,and 3 were ineffective,follow-up for 6 months showed recurrence in 4 patients,with an average length of hospitalization of 8.1 ± 2.8 days.Among the 31 patients in the internal conservative treatment group,15 were cured,5 showed significant effect,and 11 were ineffective.follow-up for 6 months showed recurrence in 11 patients,with an average length of hospitalization of 11.1 ± 4.6 days.In the bronchial arterial embolization group,a total of 183 diseased bronchial arteries were found by angiography,including 97 pure bronchial arteries,52 left and right common trunk bronchial arteries,23 Intercosto-Bronchial arteries,and 11 heterotopic origin of bronchial arteries.The responsible vessels and embolization materials for massive hemoptysis caused by different etiologies were evaluated.In the internal medicine conservative treatment group,6 patients developed hemorrhagic shock and 3 patients developed lower limb venous thrombosis.In the bronchial arterial embolization group,13 patients showed symptoms such as fever,chest tightness and chest pain,which disappeared after symptomatic treatment such as oxygen intake and analgesia.No serious complications such as transverse spinal cord injury,ectopic embolism(cerebral infarction,myocardial infarction,spleen infarction,tissue ischemic necrosis)occurred in all patients.Conclusions:Bronchial artery is involved in the blood supply of massive hemoptysis.Bronchial artery embolization in patients with massive hemoptysis is a effective,less costly and less risky treatment.
