| Objective:To observe the clinical efficacy of Pei Yuan Jie Yu acupuncture treatment of depression after stroke,and the influence of ACTH and Cort in the blood,objective to explore the neuroendocrine mechanism of acupuncture in the treatment of post-stroke depression,and to provide a new way of thinking for the treatment of post-stroke.Method:60 patients with post-stroke depression were randomly divided into treatment group and control group,and 59 cases were completed,of which the treatment group 29 cases,the control group 30 cases.The patients in the two groups received basic treatment.The treatment group was treated with Pei Yuan Jie Yu acupuncture treatment and oral fluoxetine hydrochloride treatment,once a day,treatment in 6 days then rest 1 day,a total of 6 week;the control group oral fluoxetine hydrochloride treatment,once a day,one time 20mg,the morning meal service,for the 6 week course of treatment.Two groups of patients before and after treatment with Hamilton Depression Scale(HAMD)to assess the degree of depression,using the neural function defect score scale(MSSS)to assess the degree of neurological deficit,using modified Barthel Index(MBI)to assess the activities of daily living.before and after treatment,the levels of ACTH and Cort were measured at 8 am.Finally,the data were analyzed by SPSS 20.0.Results:1.The HAMD scores of the two groups after treatment was significantly lower than that before treatment(P<0.05);and the treatment group was significantly lower than the control group,the difference was statistically significant(P<0.05).2.The MSSS scores of the two groups after treatment was significantly lower than that before treatment(P<0.05);and the treatment group than the control group decreased much more,the difference was statistically significant(P<0.05).3.The MBI scores of the two groups after treatment was significantly lower than that before treatment(P<0.05);and MBI scores in treatment group was significantly higher than the control group,the difference was statistically significant(P<0.05).4.The two group of blood ACTH and Cort levels after treatment compared with before treatment were significantly decreased(P<0.05);the ACTH level of treatment group had no significant difference with the control group,no statistical significance(P>0.05);the level of Cort in treatment group was significantly lower than the control group.The difference was statistically significant(P<0.05).Conclusion:The clinical curative effect of combined with Pei Yuan Jie Yu acupuncture treatment of depression after stroke compared with fluoxetine has better,which can improve the depression of PSD patients,promote the recovery of neurological function and improve the daily living ability of patients.The mechanism of action may be related to the improvement of blood Cort levels in patients with PSD by acupuncture.It has certain use value and prospect in clinical application. |
