| Objective:To explore the auxiliary effect of Brucea javanica oil emulsion on GP regimen chemotherapy in advanced non-small cell lung cancer(NSCLC)patients with syndrome differentiation of stagnation and pulmonary collaterals,and to evaluate its efficacy and safety.And to provide some reference for the selection of Brucea javanica oil emulsion injection.Methods:Collected in November 2015-March 2017 in fuzhou general hospital of respiratory and critical medicine and oncology department of 60 patients with met inclusion and exclusion criteria,using randomized controlled clinical trial design can be divided into the control group 30 cases(fall off 2 cases),the experimental group 30 cases(fall off 1 case).The control group was treated with simple GP chemotherapy(Gemcitabine 1000mg/m2,d1?d8+ Cisplatin 80mg/m2,dl);the experimental group was treated with Brucea javanica oil emulsion injection(30ml,dl-d10)on the basis of the control group,10 days for each cycle of treatment,21 days for a chemotherapy cycle,2 cycle for an observation period.Two groups of patients on the first cycle of treatment in the first day,the second cycle of the 10th days were taken with an anticoagulant venous blood,checking blood routine,biochemical and blood coagulation function,tumor markers,KPS score and quality of life score,evaluation of TCM syndrome score,toxic and side reaction of chemotherapy.All data were analyzed by SPSS20.0 statistical software.Results:1.Between groups before treatment in patients with general information,coagulation function,tumor markers,KPS score,determination of lung cancer patients quality of life scale(QLICP-LU)score,evaluation of TCM syndrome score and differences had no statistical significance(P>0.05).2.Blood coagulation function:In experimental group,the plasma D-dimer,fibrinogen(Fib),platelet(PLT)was lower than that before treatment,the activated partial prothrombin time(APTT)was higher than before treatment,the differences are statistically significant(P<0.05);In control group,the plasma D-dimer,Fib higher than before treatment,but there was no statistically significant difference(P>0.05),APTT,PLT was was lower than that before treatment,the difference is statistically significant(P<0.05).3?Tumor markers:CEA and cyfRA21-1 decreased significantly after treatment in both groups,the difference is statistically significant(P<0.05),The experimental group decreased than control group obviously,but the difference was not statistically significant(P>0.05).4?KPS score:The improvement rate of KPS score was 69%in experimental group and 50%in control group.The improvement rate of KPS score in experimental group was higher than that in control group,but the difference was not statistically significant(P>0.05).5?Quality of life score:the two groups after treatment of physical function,psychological function,social function,common symptoms and side effects,lung cancer specific module score and total integral were lower than before the treatment,differences statistically significant(P<0.05);Experimental physical function,common symptoms and side effects,the lung cancer specific module score and total score were better than the control group obviously,statistically significant difference(P<0.05).6?TCM symptom score:ln the total integral in the two groups of TCM symptoms,both groups of TCM syndrome score can reduce the symptoms integral,and the experimental group of TCM symptoms score more obviously than the control group to reduce total integral,statistically significant difference(P<0.05);In the improvement rate of TCM symptom,the improvement rate of TCM symptoms in the experimental group was 72%,and that in the control group was 43%,the difference was statistically significant(P<0.05).In the improvement of TCM certain symptoms,the symptom scores of cough,sputum,shortness of breath,chest pain,chest tightness of the experimental group were decreased after treatment,and the cough and sputum scores of the control group were lower than those before treatment,the difference was statistically significant(P<0.05).The experimental group in improving cough,sputum,chest tightness,chest pain and other symptoms integral aspects than the control group obviously,statistically significant difference(P<0.05).7?Evaluation of toxicity and side effects:From two groups of toxicity and side effects occurred,the number of experimental incidence of toxicity and side effects of chemotherapy is lower than the control group,but there was no statistically significant difference(P>0.05);From the point of the number of two groups of side reactions,lower incidence of toxicity and side effects of chemotherapy than controls,and the difference is statistically significant(P<0.05);From two groups of patients with specific toxicity and side effects,the incidence of bone marrow suppression is lower than the control group,while statistically significant difference(P<0.05),while in the liver and kidney function damage and the incidence of rash,experimental group is lower than the control group,the incidence of nausea and vomiting and diarrhea,the experimental group was slightly higher than that of control group,but there were no statistically significant difference(P>0.05).Conclusion:Java brucea fruit oil emulsion injection combined scheme of GP chemotherapy can better improve the patients with advanced NSCLC lung collaterals syndrome stasis resistance high blood coagulation state,improve patients' quality of life,improve the TCM symptom score,reduce chemotherapy for bone marrow suppression of auxiliary function,at the same time,it has lower levels of tumor markers,improving KPS score,alleviate kidney function damage and the trend of rash,chemotherapy and does not appear serious reaction,and It can be used safely. |
