Pronostic Value Of Preoperative Neutrophil-to-Lymphocyte(NLR) In Patients Resected For Gastric Cancer

Objective:To study the pronostic value of the relationship between the preoperative Neutrophil-to-Lymphocyte Ratio(NLR)in patients resecpted for gastric cancer and the tumor prognosis factors,to explore the assessed value of preoperative NLR in gastric cancer patients for pronositic risk,to guide the clinical treatment.Methods:This is a retrospective study in the Affiliated Hospital of Putian College from January 2013 to December 2013 with 122 cases patients data are collected,among which there are 90 men and 32 women,with an average age of 60.8 years.Collect the data of preoperative neutrophil count and lymphocyte count of patients,and calculate the NLR according to data from patients.The qualified cases can be divided into high NLR group and low NLR according to the critical value of 2.5 after checking a large number of domestic and foreign literature.Analyze the relationship between the NLR value and the age,gender,pathological type,tumor size,invasion depth,lymph node metastasis,TNM stage,HER-2 of patients and so on,and postoperative follow-up of patients,and assess the clinical value of NLR value for gastric cancer prognosis.Results:1.It is prompted by the single factor analysis of preoperative NLR value and clinical factors:the differences is statistically significant(P<0.05)between the NLR value and the age,gender,pathological type,tumor size,invasion depth,lymph node metastasis,TNM stage,HER-2 of patients.The NLR is higher with older,more Lymph node and later TNM stage;the NLR in HER-2 positive patients is higher than the negative ones.It has no obvious correlation between Gender,pathological type,tumor size and NLR(P>0.05).It shows with further Logisitic multiple factors regression analysis that age,infiltration depth,lymph node metastasis,TNM stage,its HER-2 have an impact on preoperative NLR value(P<0.05).2.The factors,tumor infiltration depth,TNM stage,lymph node metastasis,HER-2,preoperative NLR value,is statistically significant on the prognosis of patients(P<0.05),and it is opposite with age,gender,tumor size,pathological differentiation type(P>0.05).The 3 years survival rate in low NLR group was obviously better than high NLR group(P = 0.011),and the prognosis of patients with negative HER-2 better than positive ones(P = 0.02);The deeper the tumor infiltration depth(P = 0.000),the later TNM staging(P-0.000),the more lymph nodes(P = 0.006),the lower the 3 year survival rate.Conclusion:1.The level of preoperative NLR value may be closely related to age,lymph node metastasis,tumor infiltration depth and TNM stage.2.The preoperative NLR,TNM stage,lymph node metastasis,tumor infiltration depth of patients may be of great significance for prognosis.3.The preoperative NLR may serve as a simple and effective evaluation marker to the outcome of gastric cancer.
