Objective Acute pancreatitis(AP)is a common clinical Acute abdominal pain,although the treatment of AP gradually standardized in recent years,the incidence and case fatality rate still rising year by year.Find a can timely reflect clinical index of the patients is not only beneficial to the early clinical intervention,progress at the same time the patient condition and treatment plan adjustment has important significance.Has many clinical score and indicators used in the condition of AP assessment,but the indicators involved is overmuch,measuring steps and trival,spend a long time or low accuracy and sensitivity.The purpose of this paper is to study on peripheral blood alpha melanocyte stimulation hormone(?-MSH),trypsinogen activation peptide(TAP),interleukin-6(IL-6)to study the dynamic detection of its value in evaluation of AP illness severity and prognosis,and for the clinical early,accurate grasp of the illness,provides the basis for evaluation of prognosis.Methods Select emergency surgery of the second affiliated of Anhui Medical University in June 2015 to March 2016 were 97 cases of patients with acute pancreatitis(AP)as the research object,the mild group 56 cases,41 cases of severe group.All patients admitted to hospital after 1,3,5,7 days extracting peripheral venous blood,using enzyme-linked adsorption method for quantitative determination of serum ?-MSH,TAP,IL-6 level,severe patients admitted to hospital to give an acute physiology and chronic health evaluation ?(APACHE ?)?Sequential organ failur assessment(SOFA)and Ranson score.And then for data collection,the mild group and severe group,severe group multiple organ dysfunction syndrome(MODS)and not multiple organ dysfunction syndrome(MODS)statistical comparison and ROC regression curve analysis.Results Through the study found that compared with mild group,severe group of peripheral blood of ?-MSH hospital a week at lower levels,IL-6 at higher levels,three days prior to admission in severe group of TAP at a higher level,mild and severe group were significant difference(P < 0.05).In the prediction of severe acute pancreatitis(SAP)in MODS,?-MSH sensitivity,specific degree,area under curve were 80.3%,72.0%,0.848;SOFA score sensitivity,specific degree,area under curve were80.2%,72.4%,0.819;Ranson score sensitivity,specific degree,area under curve were76.7%,72.1%,0.818;APACHE-? score sensitivity,specific degree,area under curve were 80.1%,70.3%,0.789;TAP sensitivity,specific degree,area under curve were62.8 %,70.1%,0.675;IL-6 sensitivity,specific degree,area under curve were75.3%,60.1%,0.693;?-MSH,TAP and IL-6 combined sensitivity 87.0% and specific degrees75.0%,the area under the curve was 0.906.Conclusion Peripheral blood of ?-MSH,TAP,IL-6 levels in acute pancreatitis illness severity evaluation has certain clinical value,can be used to evaluate AP illness severity.In evaluating the prognosis of patients with respect,?-MSH sensitivity,specificity and accuracy were higher than the TAP and IL-6,in addition,the three common detection has higher sensitivity,specificity and accuracy.
