Objective To elucidate the pathogenesis of severe acute pancreatitis (SAP) and the role of human urinary trypsin inhibitor (UTI) in the pathophysiology of SAP for new effective treatment.Methods UTI was highly purified from the urine of patients with acute pancreatitis by ammonium sulfate precipitation , Sephadex G-75 and SP- Sephadex C-25 column chromatography. Fifty-four Wistar rats were selected and divided randomly into three groups: normal control group (NC,n=18),servere acute pancreatitis group (SAP,n=18) and UTI test group (UTI,n=18). Under surgery, SAP models were established by injecting 5% sodium taurocholate (NaTc) with dosage of 0.1 ml per 100g weight under pancreatic capsule. NC group were injected same dosage normal saline and UTI group were given UTI with dosage of 1000IU ml per 100g weight 10 minutes before establishing SAP models in same manner. Light microscopy on the pancreatic pathologic tissue and electron microscopy on the pathologic changes of ultrastructure of the pancreatic acinar cells were carried out continuously. The changes of following indexes those are amylase plasm calcium, thromboxane A2 (TXA2), prostaglandin I2 (PG I2), interleukin-6(IL-6) were dynamically determined. Also the living time and the volum of ascitic fluid were observed. Results1. The purification and high valence UTI was harvested.2. The changes of pathology were observed just one hour after the injection of 5% sodium taurocholate. In SAP group it was more seriously than NC and UTI group. And the volum of ascitic fluid was significantly more than the other two groups. Also the living time were significantly shorter than the others groups.3. In SAP group the level of serum amylase, TXB2, 6-Keto-PGF1a and IL-6 were significantly higher than those of NC and UTI groups. But the concentration of serum calcium were significantly lower than those of the other two groups.Conclusion 1. The highly purificated UTI was harvested.2. The rat SAP model was successfully reproduced with injection of 5% sodium taurocholate.3. The changes of Intracellular calcium ions, TXA2, PGI2 and IL-6 might play critical roles in rat SAP. 4. UTI has beneficial effect on SAP.
