Objective: To investigate the association between serum uric acid(UA)and the severity of coronary artery disease in patients with unstable angina pectoris(UAP).Methods: 280 patients include 138 men and 132 women were hospitalized in our department(according to the symptoms for unstable angina)and they were assigned into unstable angina pectoris group(188 cases)and control group(82 cases)by coronary angiography results and the patients' complain of symptoms and myocardial necrosis markers.Basic data including gender,age,history of hypertension,history of diabetes,history of smoking,laboratory test(blood routine,biochemical data,coagulation function)and the result of coronary angiography(Gensini score)was collected.The association between the serum uric acid level and the severity of coronary artery disease in different groups was analyzed.Patients with unstable angina pectoris had interviewed to collect cardiovascular events(including recurrent unstable angina,new myocardial infarction,ischemic arrhythmia,cerebrovascular events,sudden cardiac death,congestive heart failure)during the six months to two years after treatment in our hospital.The data was analyzed by the multivariate unconditional Logistic regression model whether the serum uric acid level was an independent predictor of adverse cardiovascular events.Results: 1.The factors of patients with unstable angina pectoris were higher than the control group on indicators of the proportion of men,average age and rate,the prevalence of diabetes and the history of smoking and the prevalence of hypertension;unstable angina pectoris patients were significantly higher than the control group on indicators of the uric acid and the low density lipoprotein(LDL)level(333.07 ±65.95vs284.02±68.29,P=0.001;2.73±0.85vs2.47±0.84,P=0.024),the high density lipoprotein(HDL)level(1.03±0.26vs1.21±0.36,P=0.001)were lower than that of control group.2.The uric acid level of the double vessel disease group was higher than it in single vessel group and control group,(326.89 ± 48.99vs299.75 ± 60.24,P=0.008;326.89 ± 48.99vs284.02 ± 68.29,P=0.001).The uric acid level of the multi-vessel disease group was higher than it in double vessel disease group and single vessel disease group and the control group(365.35 ± 62.55vs326.89 ± 48.99,P=0.001,365.35±62.55vs299.75±60.24,P<0.001;365.35±62.55vs284.02± 68.29,P<0.001).3.The uric acid level was positively correlation with Gensini score(r=0.548,P<0.001).The high density lipoprotein level was negatively correlation with it(r=-0.088,P=0.043),The low density lipoprotein was positively correlation with it(r=0.046,P=0.001).4.the factors predicting cardiovascular events in patients with unstable angina pectoris were uric acid(OR=4.431,95%CI 1.003-1.017,P<0.001)and HDL(OR=0.869,95%CI,P=0.022).5.The age was positively correlated with the uric acid level.(r=0.265,P=0.0.041).The Cr level was correlated with the uric acid level(r=1.476,P=0.023).6.There was different incidence rate of cardiovascular events in different levels of uric acid group(57.14%vs60.86%vs62.22%vs65.96%,P<0.001),there was significant difference between the incidence of unstable angina group in different levels of uric acid(34.69%vs36.96%vs39.13%vs40.43%,P<0.001),the incidence of congestive heart failure were different in different levels of uric acid group.(18.37%vs19.57% vs20.02%vs21.27%,P=0.042).The probability of all adverse cardiovascular events,unstable angina and congestive heart failure increased with the increase of uric acid level.Conclusion: 1.The level of uric acid in patients with unstable angina pectoris was significantly higher than that in the control group.The high level of serum uric acid was one of risk factors of unstable angina pectoris.2.The level of uric acid was correlated with the severity of coronary artery disease,and it increased with the severity of coronary artery disease.3.Serum uric acid was positively correlation with the occurrence of cardiovascular events in patients with unstable angina pectoris.
