Objective: 1.To evaluation of left and right ventricle structure and function characteristics in elderly patients with atrial fibrillation by real-time three-dimensional echocardiography and three-dimensional speckle tracking imaging.2.To evaluation of right ventricle structure and function characteristics in elderly patients with atrial fibrillation by real-time three-dimensional echocardiography and tricuspid annular plane systolic excursion.Methods: 80 patients(average age of 81.2 years)with lone atrial fibrillation(LAF)whose medical history more than 20 years were used as the experimental group(group B),other 80 healthy persons(average age of 79.9 years)were used as the control group(group A).1.The parameters of the conventional two-dimensional echocardiography were obtained respectively;the area of left atrial end-diastolic(LAA),left ventricular end-diastolic diameter(LVDd),left ventricular ejection fractions of Simpson(LVEF),end-diastolic thickness of ventricular septum(IVSTd),left ventricular posterior wall thickness diastolic(PWd),left ventricular end-systolic diameter(LVDs),the area of mitral regurgitation(MRA)and MRA/LAA,the parameter of left ventricular diastolic function E/E',the area of right atrial end-diastolic(RAA),The basal parts of right ventricular diameter in end-diastolic(Basal RVd),right ventricular fractional area change(RVFAC),the area of tricuspid regurgitation(TRA)and TRA/RAA,pulmonary artery systolic pressure(PASP),tricuspid annular plane systolic excursion(TAPSE),therate change of inferior vena cava(?IVC),the parameter of right ventricular diastolic function e/e,.2.The parameters of the real-time three-dimensional echocardiography were obtained respectively :left ventricular end-systolic volume(LVESV),left ventricular end-diastolic volume(LVEDV),left ventricular ejection fractions of three-dimensional(RT-LVEF),right ventricular end-systolic volume(RVESV),right ventricular end-diastolic volume(RVEDV),right ventricular ejection fractions of three-dimensional(RVEF).3.The parameters of the three-dimensional speckle tracking imaging were obtained respectively : left ventricular general longitudinal strain(LVGLS),left ventricular general circle strain(LVGCS),left ventricular general radial strain(LVGRS),left ventricular general area strain(LVGAS).Results:1.Conventional parameters: Compared with control group,there were no significant difference between group B and group A of LVDs,IVSTd,PWd(all P>0.05);LVDd,LAA,MRA,E/E',BasalRVd,TRA,RAA,PASP,e/e',MRA/LAA,TRA/RAA increased significantly through the comparison of group B with groups A(P<0.05 or P<0.01),while RVFAC,TAPSE,?IVC,LVEF decreased significantly(all P <0.05).2.Three-dimensional parameters: LVEDV,LVESV,RVEDV,RVESV increased significantly through the comparison of group B with groups A(P<0.05 or P<0.01), while the RT-LVEF,RVEF decreased significantly(P<0.05 or P<0.01).3.Three-dimensional Speckle Tracking Imaging parameters: The absolute value of LVGLS,LVGCS,LVGRS,LVGAS decreased significantly through the comparison of group B with groups A(P<0.05 or P<0.01).Conclusion:1.The elderly patients with atrial fibrillation with long-term ventricular rate poor controlling will change in cardiac structure and function,the performance of the double atrium chamber increased significantly.The increased of volume and decreased and function of left and right ventricular.Tricuspid regurgitation was more obvious than mitral regurgitation.The increased of pulmonary artery systolic pressure slightly to moderately.2.Real-time three-dimensional echocardiography and Three-dimensional speckle tracking imaging can be assessment of left ventricular volume and ejection fraction in patients with elderly atrial fibrillation.To provide a comprehensive basis for clinical diagnosis,treatment and prognosis.
