| Background:In recent years,the incidence of papillary thyroid carcinoma has increased in the world.especially in the case of papillary thyroid carcinoma.Although papillary thyroid microcarcinoma is slow and does not appear to have clinical symptoms for a lifetime,a small number of patients have been shown to have aggressive behavior and have an early stage of lymph node and distant metastasis.Therefore,it is very important to detect and diagnose papillary thyroid carcinoma.The number of studies discussing the aspect ratio relationship with the risk of malignant thyroid nodules,and there are many studies based on size classification of nodules,theaspect ratio of the diagnostic value in different sized nodules,bu the conclusions of these studies are controversial.Objective:A meta analysis of diagnostic tests is use to eveluate the diagnostic value of A/T>1 in the diagnosis of papillary thyroid carcinoma.In order to provide a more reliable basis for the diagnosis of thyroid papillary carcinoma in A/T >1.Methods:Computer comprehensive online retrieval of Pubmed and Embase two databases,access to the relevant documents published during January 1990 to December 2016 in English and Chinese.After the end of the search,two reviewers independently implemented the inclusion criteria of the literature and screened them,and then used Meta-Disc 1.4 software to complete the Meta analysis.Results:Through reading into the literature,we finally got 8 documents from Korea and China in two countries,All were retrospective studies.These documents include the number of cases in 4778 cases,nodules > 10 mm accounted for 24%(1150 cases),nodules≤10mm accounted for 78%(3628 cases).The Meta analysis results showed that: the diagnosis sensitivity of A/T > 1 in nodules> 10 mm and nodules≤ 10 mm were 0.38,0.65.The specificity was 0.93 and 0.80,respectively.The combined positive likelihood ratio was 3.96 and 3.74,and the negative likelihood ratio was 0.75 and 0.38,respectively,and the diagnostic odds ratio was about 6.18 and 12.38,respectively.Sensitivity and specificity were calculated and the area under the SROC curve was calculated.The area under the curve(AUC)of A/T >1 in the diagnosis of >10mm was 0.8165.the area under the curve(AUC)in the diagnosis of nodules≤ 10 mm was 0.8394.Conclusion:As our vision,A/T > 1 as a feature of PTC ultrasound diagnosis,A/T > 1 for the diagnosis of PTC is less than or equal to 10 mm the specificity,sensitivity and accuracy are relatively high,The specificity and accuracy of PTC > 10 mm are relatively high,but the sensitivity is low.Therefore A/T > 1 is the maximum diameter of less than 10 mm PTC reliable diagnosis ultrasound features,can be used as the main diagnostic criterion.For the maximum diameter of 10 mm PTC,A/T > 1 is not suitable for its main diagnostic criteria. |
