Objective 1).To find the risk factors for recurrence of chronic rhinosinusitis with nasal polyposis(CRSw NP),In order to provide a reference for preventing the recurrence of nasal polyps;2).Counting the tissues eosinophils in recurrent nasal polyps,to explore the diagnostic value of eosinophils in the treatment of recurrence nasal polyps.3).Combine with literature to elaborating Molecular biology,immunological mechanism and research status of eosinophils involved in the process of nasal polyps.Methods This was a retrospective study that used data collected prospectively from patients with bilateral nasal polyps who had undergone endoscopic sinus surgery at The second affiliated hospital of Jilin university during October 2015 to October 2016.All the patients satisfied the diagnostic criteria,had complete medical records,before operation examined the blood routine,sinus CT,allergen,and send polyps tissue to the pathology intranperative period,counting the tissue eosinophil,regular medication after discharge,regular follow-up and checking the electronic nasharynx.Statistical analysis was performed by using SPSS version 20.0(IBM Corp.,Armonk,NY),the chart was Applied by using Graph Popad Prism 6.0.Results 1)There was no difference between the recurrence group and the non-recurrence group in allergen species(p=0.764)and the total Ig E(p=0.311).2)Between the recurrence group and the non-recurrence group,there was no significant difference in the number and percentage of peripheral blood eosinophils,and there was also no significant difference between neutrophilto-lymphocyte(NLR)ratio and platelet-to-lymphocyte(PLR)in peripheral blood.3)There was no significant difference in the degree of obstruction between the ethmoidal sinus and the ostiomeatal complex(OMC)between the two groups,but The ratio of the CT scores for the ethmoid sinus and maxillary sinus(E/M ratio and the maxillary sinus score had the highest were significantly different,and the maxillary sinus score is an independent risk factor for recurrence of chronic sinusitis in patients subjected to endoscopic sinus surgery.4)Asthma and tissue eosinophil count were different in the non-recurrence group and recurrence group,and both were independent risk factors for recurrence of nasal polyps.5)the area under the ROC curve,maxillary sinus score is 0.761,tissue eosinophil count is 0.636,however tissue eosinophils have a better distinction.Conclusion 1)Tissue eosinophilia markedly correlated with polyp recurrence and could be used to predict disease recurrence.2)Maxillary sinus score and E / M score had an effect on nasal polyp recurrence.The maxillary sinus score was an independent risk factor for recurrence of nasal polyps,but it was not easy to distinguish between the recurrence group and the non-recurrence group.3)Asthma for the recurrence of nasal polyps risk factors,accompanied by asthma,nasal polyps should be treated regularly after surgery,close follow-up.4)There were no significant differences in allergen species and total Ig E between recurrence group and the non-recurrence group.5)Peripheral blood,while easy to obtain,may not reflect the inflammatory changes observed locally in nasal polyps.
