Objectives:Maxillary sinus septa are bone walls arising from sinus walls that may divide the sinus into two or more cavities.Maxillary sinus septa may increase the risk and difficulty of maxillary sinus floor elevation surgery,and even lead to perforation of schneiderian membrane.Aim of this investigation is to evaluate the anatomical regulation of maxillary sinus septa,sinus floor and schneiderian membrane.To provide some preoperative direction for maxillary sinus floor elevation surgery that may reduce the complication and difficulty of surgery.Methods: This investigation collected 239 64-slice computed tomography data during the time from 2013.01 to 2015.12 in department of oral and maxillofacial surgery at Shanghai Ninth Hospital affiliated to Shanghai Jiao Tong University School of Medicine.All data were measured by Mimics15.0(Materialize Company,Belgium)and analyzed by SPSS21.0.To evaluate prevalence,localization,morphology and height of maxillary sinus septa.The thickness of the mucosa and the morphology of maxillary sinus floor had been measured together.Sex,age,tooth losing,periodontitis and smoking were analyzed as influential factors.Results: A total of 239 patients and 478 maxillary sinus had been measured,with 144 patients and 201 maxillary sinus had maxillary sinus septa.The prevalence of sinus septa was 60.3% by patients and 42.1% by sinus.There was no statistically significant difference of prevalence in different group divided by sex,age,tooth losing,periodontitis or smoking.Most of the septa distributed at molar area(46.4%)and premolar area(30.0%),and most of the septa located at sinus floor(68.4%).In morphology of septa,serrate shape septa(44.3%)and blade shape septa(34.2%)were mostly seen,the raised shape septa(16.5%)and oval shape septa(5.1%)were relatively infrequent.The mean length of septa was(9.73±4.42)mm,mean width was(4.63±2.72)mm,and mean height was(4.80±3.01)mm.The mean thickness of mucosa around septa was(1.40±0.78)mm.The mean thickness of schneiderian membrane was(1.49±0.86)mm.There was a statistically significant difference(P=0.014)between the thicknesses of mucosa near septa and the mean thicknesses of schneiderian membrane in same sinus.There was no statistically significant proportional relation between dimensions of septa and thickness of mucosa.The morphology of sinus floor were mostly show as concave shape(58.2%)and flat shape(26.2%),the convex shape(10.9%)and atypical shape(4.8%)were rare.There wass a rate of 26.4% patients show different shape of sinus floor in two sides.Conclusions:Multi-slice computed tomography can be useful research data with the advantages of less artifacts,clear 3D reconstruction effect,and brilliant ability of discerning both bone and soft tissue.In this investigate.The prevalence of sinus septa was 60.3% by patients and 42.1% by sinus.There were 52.3% sinus septa(locate at sinus floor,premolar and molar area in sinus)may influence the maxillary sinus floor elevation surgery.Clinical dentists should pay attention to maxillary sinus anatomy before the sinus lift surgery planning,and pay attention to both sides of sinus if needed.The thickness of mucosa was thicker around septa in this investigate,which worth further research.
